Persistent high eosinophils
My father, 63 male, has Type 2 Diabetes, Hypertension and CAD. He works with sheep (animal exposure) Had leg cellulitis in Aug 2025 (now settled). He is otherwise keeping well. Eosinophils were already high in 2019 & 2022 (not investigated then). Recent reports again show marked eosinophilia (AEC earlier 4.96, later ~3.13; up to 35%). IgE is very high (~2650). No known allergies. Other findings: Mild anemia (Hb 11.6), clear iron deficiency (Ferritin 18.8, saturation 6%), B12 normal, Coombs negative. Seen by Haematology and Immunology. Doctor called it an “incidental finding” as he has been keeping well. Advised USG and possible genetic tests later. Questions: 1. What are the most likely causes (long-standing high eos + high IgE + sheep exposure)? 2. Since he has been well for years, do we need to investigate urgently/promptly or can we take it slowly step by step? 3. Which tests should we prioritise now? 4. Any risk of silent organ damage even if he feels fine? Thank you.

Persistent eosinophilia in this range needs structured evaluation even if he feels well. I would repeat CBC with absolute eosinophil count and review the trend, then do stool ova/parasite testing (guided by exposure) and a careful medication review. Allergy/parasitic causes are common, but drug-related and clonal blood disorders must also be excluded. Depending on symptoms and examination, chest X-ray (or CT if indicated) and review with a haematologist/immunologist are appropriate, because silent organ involvement can occur.
