High fever that is not settling despite paracetamol and two days of antibiotics suggests that this may not be a simple throat infection, or that the antibiotic choice may not be adequate. A sore throat with persistent high fever often requires a proper clinical examination and sometimes additional tests (like throat swab, CBC, viral screening) to rule out bacterial vs viral causes.
Next Steps
Since the fever is touching 103°F and not subsiding well with medicines, I would strongly recommend getting him evaluated by a doctor in person today itself. He may need a change of antibiotics or supportive treatment. In the meantime, keep him well hydrated, continue paracetamol every 6 hours as required (but do not exceed the maximum daily dose), and monitor for warning signs like difficulty breathing, drowsiness, or rash.
Health Tips
Please do not continue antibiotics on your own without medical supervision, as the choice and dose need to be tailored. A timely consultation will prevent complications and give him faster relief. If convenient, you can also book a consultation with me on Practo for a quick review of his case and guidance on the right treatment.

I understand your concern. From your description, your son has been having a sore throat with fever that has now persisted for 2–3 days and is spiking up to 103.8°F despite starting azithromycin (Azee 500) and giving paracetamol (Dolo 650).
High-grade fever not settling with antipyretics(dolo 650), especially beyond 48 hours of antibiotics, suggests that we need to reassess the diagnosis and treatment plan. This could be a severe bacterial throat infection, tonsillitis, or even a viral illness that won’t respond to azithromycin. Sometimes resistant bacteria, or infections like streptococcal throat, require a different class of antibiotics.
What you should do now:
I strongly advise you to consult through video call with me for proper detailed history and prescription.
The above and below ones are just differentials not a proper diagnosis
Or else
Continue paracetamol every 6–8 hours as needed (do not exceed the daily maximum dose). As fever is not settling with oral medications might need to start him on IV Paracetamol.
Ensure he is well hydrated, takes plenty of fluids, and gets rest.
Tepid sponging (wiping the body with lukewarm water) can help lower fever when it’s very high.
Most importantly: since the fever is not coming under control and it’s already the 3rd day, . He may need a throat examination, blood tests, or even a change in antibiotics.
If he develops difficulty in breathing, severe throat pain preventing swallowing, dehydration, or persistent high fever >104°F despite medicines, please do not delay—visit the emergency room immediately.
Hello, I am Dr. Romain Rajan, General Physician.
I understand your concern â your son has had a sore throat with persistent fever despite antibiotics and paracetamol. Ongoing fever above 102 degrees and worsening symptoms require careful evaluation
â Advices
⢠Ensure he stays well-hydrated and gets adequate rest
⢠Use paracetamol (Dolo 650) as advised but do not exceed the recommended daily dose
⢠Avoid self-starting new antibiotics without a doctorâs guidance
What to Monitor
⢠Watch for increasing fever, difficulty swallowing, severe throat pain, or swelling of neck
⢠Note any breathing difficulty, rash, or lethargy
Management
Since he has persistent high-grade fever despite taking Azee 500mg and paracetamol, he needs urgent in-person evaluation. The doctor may need to check for strep throat, other bacterial infections, or viral complications and decide if a change of antibiotics or further tests are needed
When to Consult a Doctor Immediately
⢠Fever above 103 degrees not responding to paracetamol
⢠Difficulty breathing, severe throat swelling, or inability to swallow liquids
⢠Signs of dehydration, confusion, or persistent vomiting
Every patientâs situation is unique. For accurate diagnosis and effective treatment, it is important to examine your son and review his medical history in detail
You can reach out to me directly for this case, and also connect with me in future for any medical consultation

For your son as its high grade step ladder type of fever , it may mostly be typhoid in first place as monsoons season ! So i suggest give high dose tab . Paracetamol 1.2gm po bd at home and get blood test of widal , dengue and malaria [ fever profile ] . And mostly I feel your son must be admitted in hospital for injectable IV management.
Look if neglected it will cause more harm to your son .
Next Steps
get blood test and join in hospital asap .
Health Tips
be caution of bleeding gums , blood like rashes and high spikes of fever .
