I have my b12 levels 199 pg/ml with symptoms brain fog, dizziness when I walk outside. I am fine whenever I am inside home. I also have tongue ulcers due to b12 deficiency. I took Methycobol 500 tablets twice a day for one month. But still no improvements in symptoms. Then I switched to Neurokind OD for two days. But after switching to Nurokind OD my symptoms are getting worst. I am getting anxiety, high heart beats, more dizziness after taking Nurokind OD. So I stopped it and switched back to Methycobol 500 two tablets per day. Please help me regarding this as my symptoms are not improved yet.

Brain fog, dizziness, tongue ulcers have multifactorial causes. Only Vit B12 correction will not address your symptoms.. need deeper evaluation for complete treatment. Kindly consult.
Kindly share your vitamin b12 levels , which ever you have done last ... Smoking history will be important.. taking too much b12 without tracking it can also lead to symptoms like tachycardia ( high heart rate) , anxiety , sudden muscle twitchings especially in eyes
Also need for more history evaluation is there to rule out other probable causes of such symptoms

If no allergic to multivitamins take
-tab Cytoplex MV OD for 2 months.
Sometimes oral doesn't work due to malabsorption or deficiency of Intrinsic factor so take it in injectable form
-Inj Bee12 alternative days for 10 days sufficient
Symptoms improve later
Health Tips
For better evaluation , consult me with practo online with detailed history.
Vitamin B12 deficiency has been more common, due to poor food habits, most of them eat junk foods which lacks vitamins required for our body,it is best to eat home cooked food for better nutrients, eating regular outside food can lead to multiple deficiency disorder, children aswell as elders should avoid having food outside regularly, home cooked with plenty of fruits, vegetables, pulses, eggs, milk, lean meat etc gives all the vitamins and minerals required for our body.
Next Steps
Now the patient has b12 deficiency, better to start with methyl cobalmiine supplements 2 times daily for 15 to 1 month.later blood test done to check for any changes..if no changes seen them advised b12 injection untill the symptoms subside, also advised to take b12 rich fruits and vegetables regularly.
Health Tips
Avoid having outside food regularly, prefer home cooked food.
Your B12 is borderline low and symptoms can take a few weeks to improve, so don’t worry. Avoid switching medicines frequently and continue one B12 supplement regularly; also check CBC, Vitamin D and thyroid once.
Next Steps
If severe palpitations, fainting, chest pain or worsening dizziness happen, seek urgent care. Please consult me for proper diagnosis and treatment.
U need methylcobalamine 1500mcg o e after dinner for two months. Neurokind od contains some methylcobalamine 1500 only. It's not possible to symptoms becomes worst. It's called anxiety.
500 is the dose for <12 years. Polybion lc syr10 ml after dinner for 4 weeks.
Your Vitamin B12 = 199 pg/ml → this is borderline low / deficient range, especially with symptoms.
Your symptoms:
• Brain fog
• Dizziness outdoors
• Tongue ulcers
• Anxiety
• Palpitations after new supplement
This suggests symptomatic B12 deficiency, but there are 3 important clinical points:
Next Steps
You need structured evaluation, not random switching.
Step 1: Do these tests
• CBC (look for MCV elevation)
• Serum ferritin
• Vitamin D
• TSH
• Homocysteine (if possible)
• Peripheral smear
Step 2: Consider injectable therapy
If symptomatic with level 199:
Typical regimen:
• 1000 mcg IM daily × 5–7 days
• Then weekly × 4 weeks
• Then monthly
In symptomatic cases, injections work faster and more reliably.
Step 3: Continue oral only as maintenance later.
Health Tips
✔ Tongue ulcers improve first — neurological symptoms take 6–12 weeks.
✔ Don’t expect brain fog to clear in 2–4 weeks.
✔ Avoid constantly monitoring symptoms — anxiety worsens dizziness.
✔ Ensure protein intake is adequate.
✔ Hydrate well.
Important:
If you have:
• Numbness in feet
• Balance issues worsening
• Electric shock sensation in legs
• Severe gait disturbance
You need urgent neurological evaluation.
⸻
Your worsening after Neurokind OD is most likely supplement sensitivity.
Your incomplete recovery after one month oral therapy suggests either:
• Inadequate absorption
• Inadequate dosing
• Another coexisting deficiency
This is manageable — but needs proper correction plan.
If you’d like, we can review your full lab profile and symptoms systematically in an online consultation and design a structured B12 correction plan so you recover completely instead of experimenting with supplements.
Neurological symptoms (brain fog, dizziness) can take weeks to months to improve even after starting treatment. One month is sometimes not enough.you need to dose adjustment and increase the dose from 500 to 1000 mcg because orally b12 tab.has low absorption. Also some routine testing-CBC (for anemia, MCV),Serum folate,Iron profile,Vitamin D,Thyroid function test
Serum homocysteine or MMA.consult me on practo for further management and evaluation.
For your persistent symptoms, continue tab methycobol, take mushrooms, fortified cereals, eggs, curd, cheese, nuts, in your diet.
Get a thyroid function tests, ecg, cardiac echo done to exclude any other underlying cause for your problem and share results.
Hello,
Your Vitamin B12 level (199 pg/ml) is borderline low and can definitely cause symptoms like brain fog, dizziness, anxiety, palpitations, and tongue ulcers. However, oral tablets alone sometimes take longer to improve neurological symptoms, especially if there is absorption difficulty.
A few important points:
No improvement after 1 month is not unusual — nerve-related symptoms may take 6–12 weeks or longer to recover.
Neurokind OD contains additional vitamins which sometimes cause palpitations or anxiety in sensitive individuals, so switching back to methylcobalamin is fine.
For faster recovery, many patients benefit more from B12 injections initially rather than tablets.
Next Steps
Suggested approach
Inj Methylcobalamin 1000 mcg IM alternate day for 1–2 weeks, then weekly for 4–6 weeks.• Continue oral methylcobalamin 1500 mcg daily after injections.• Add folic acid if not already included.
Also check other causes of dizziness/brain fog:• CBC (for anemia)• Vitamin D level• Thyroid (TSH)• Blood sugar• Blood pressure (sitting and standing)
A B12 level of ~199 pg/ml can definitely cause symptoms like brain fog, dizziness, mouth ulcers, and anxiety.
Neurological symptoms often take weeks to months to improve, even after starting treatment, so lack of immediate relief is common.
Some people do feel transient anxiety, palpitations, or dizziness after certain vitamin combinations, which usually settles after stopping them.
Next Steps
If symptoms are worsening, persistent, or affecting daily functioning, reassessment is important.
It is also important to rule out associated issues like anemia, folate deficiency, thyroid problems, or blood pressure variation.
Health Tips
I would advise a proper clinical consultation for targeted evaluation rather than frequent medication changes on your own.
Exact cause of ongoing symptoms cannot be determined without examination and reports.
Cause could be other than b12 defficiency, as 197 is not v low to give symptoms you are referring to, and defficiency is corrected by now irrespective of any brand
