Male Masturbation

DefinitionMasturbation is deliberate self-stimulation, which affects sexual arousal.In Males:About 94 percent males are involved in masturbation which leads to orgasm. Some individual do not masturbate because they do not have sufficient sex drive. There are some slow reacting individuals who find it impossible to effect orgasm in masturbation. Where adults discovered the activity in children, they are reprimanded or punished, made a public exhibition of the offence which can upset the child’s peace of mind.Masturbation provides the chief source outlet in early adolescence.There are few males who masturbate only once or twice in their lives;  and other are who have frequencies that may average seven to twenty or more in a week for long periods of years. There are few males who are still masturbating at 75 years of age.Masturbation after marriage is confined to those periods when wife is away. Sometimes masturbation is product of the fact that wife does not want sexual relations as frequent as male would like to have, or that periods of pregnancy, menstruation or illness interfere with regular intercourse.There are definite taboos against masturbation, with the explanation that it will drive you crazy, give pimples, make one weak, bend the penis, cause impotence, affect vision or do some other harm, more often masturbation is simply rejected because it is considered unnatural.Masturbation usually does not continue for more than a minute or two. Some individuals achieve orgasm in10-20 seconds. In males, masturbatory techniques are largely manual. Some boys masturbate by moving penis against a bed or against some other object. All males experience sexual fantasies during masturbation. Some use erotic pictures for stimulation during masturbation. Many individuals find some additional stimulation in observing their own genitalia.Boys learn to masturbate through friends or literature. Frequency of masturbation is inversely proportional to nocturnal emission. Males with high frequency of masturbation may have low rate of nocturnal emission and vice versa.Masturbatory activity does not do any harm to the individual. It is the conflict, fearful social disgrace, guilt-feeling that harms. It ultimate sexual capacities, occasionally attempting suicide. On the other hand masturbation provides a regular sexual outlet which alleviates nervous tension and body live a balanced life. There is no  evidence to prove that masturbation interferes with high mental physical or moral efficiency.There is tremendous individual variation in the capacity to engage in this sexual activity without undue fatigue. Therefore it is not possible to define excess. Secondary the AutonomicNervous system has control over the sexual activity. Like salivation or perspiration the response is adequate according to the stimulation.For most males, coitus is primary and masturbation is secondary. For young people masturbation is the chief source of sexual outlet up to the time of marriage. Masturbation is an alternative for coitus.

Erectile Dysfunction: Causes and Treatment

Erectile dysfunction (ed) occurs when the man is unable to achieve or maintain his erection of penis to meet his or his partner's sexual satisfaction. The problem is often faced by newly married men. Fortunately with experience, most of them are able to overcome this problem. Again it is increasingly common with increased age. So, what are the causes. In some male, it is psychological, caused by anxiety, depression, stress, marital disharmony etc. Use of alcohol and smoking will aggravate the problem. Psychological causes can happen at any age but more common at younger age. But in some cases, it is associated with serious diseases, especially at old age. It has been seen that in apparently healthy male ed is the first sign of heart disease. So, if you have ed, make sure that i's not caused by your heart problem. Again diabetes is another important cause. Excessive weight gain will also cause hormonal imbalance and thus problems in erection. Some hormone may also cause ed like thyroid, prolactin and even testosterone (male sex hormone). If you have chronic diseases like liver or kidney disease, it may also lead to ed. Diseases of nerves and spines should also be kept in mind. Sometimes, infections or tumour in private parts (prostate, testis etc) will also add to ed. Other diseases of penis like abnormalities in blood vessels can also be responsible, especially in young guys. Apart from these, some surgeries in abdomen and some drugs (like drugs to lower blood pressure, antidepressants, hormonal drugs etc) will also cause ed.Now, what you can do? if you want to suffer silently, you will land up in other problems like problem in sexual life, problem in family relationships, depression and even inability to make your partner pregnant. But do all men with ed have to go to doctor? initially, the men should try life style modifications like avoiding smoking and alcohol; avoiding stress and anxiety; making good understanding with partner and also reducing weight. The partner should also have sympathetic attitude to the husband who is having ed.But if still the issue is not resolved you have to consult doctor. It's not a mater of hesitation or embarrassment, as you may think. Rather it is needed to ensure that you are healthy. So, your doctor will talk to you, will check you and try to find out if any cause is there or not (like heart disease, diabetes, hormones etc). He may order some basic tests to ensure that everything is alright. After that he will advise you the treatment. If the cause is found, treatment of the cause (like diabetes) may cure ed. Even if no cause is found, ed can be treated by various ways.But should you try the drugs shown in television or newspaper? remember, drugs used in ed have important side effects and we advise drugs only when it is absolutely necessary. In most cases simple measures like stress reduction, exercise help. Sex counselling of both the partners helps a lot. And if still ed persists, we advise drugs. There are variety of drugs, all are having own advantages and disadvantages. Particularly sildenafil group of drugs can affect heart and retina. So, we always practice caution before prescribing such drugs. And when patients are advised such drugs we advise to inform us when he feels eye problems, chest pain, fainting or even painful persistent erection of penis (" priapism").There are other drugs that can be applied directly over penis. Testosterone group of drugs are prescribed only in selective cases as they have serious side effects and even they can reduce the size of your testes. In some cases where drugs cannot be given or drugs fail, we advise penile implants or artificial pump to maintain erection. Finally in very few cases, where blood vessels of penis is blocked, we can cure it by some operations. So, if you feel ed, don't get depressed. At the same time don't feel embarrassed to take medical help. Today's embarrassment may harm you in future. So, get cured and enjoy your personal life.

ERECTION OF PENIS.  Penile erectile tissue is contained in three corporal bodies: two dorsal corpora cavernosa and a single corpus spongiosum. All the corpora surrounded by a thick fibrous sheath, the tunica Albuginea. All the corpora contain numerous cavernous spaces separated by trabeculae, composed by trabeculae, composed of thick bundles of smooth muscles Corpus spongium contains fewer muscles cells. The walls of cavernous spaces are covered by endothelial cells resembling those found in the blood vessels. Sexual activity, initiated by parasympathetic stimulation via hypogastric nerve, promotes the release of Nitric oxide (NO) from the endothelium of corpora. NO activates enzyme guanylate cyclase, allowing production of cyclicGuanesine monophosphate (cGMP) which causes relaxation of smooth muscles. As arterioles and the muscles relax, lacunar spaces are filled with blood, increasing the pressure on the veins and occluding the venous return and causing the penis to become erect. If there is no further sexual stimulation, cGMP is metabolized by phosphodiesterase 5 (PDE5) resulting loss of erection.After ejaculation sympathetic tonic discharge resumes adrenergic never terminals activate adrenergic receptors, resulting in contraction of smooth muscles around the sinusoids and arterioles. Penile pain can affect the base, shaft, or head of the penis. It can also affect the foreskin. An itching, burning, or throbbing sensation may accompany the pain. Penile pain can be a result of an accident or disease, and it can affect males of any age.The pain can vary depending on what underlying condition or disease is causing it. If you have an injury, the pain may be severe and occur suddenly. If you have a disease or condition, the pain may be mild and may gradually get worse.Any type of pain in the penis is a cause for concern, particularly if it occurs during an erection, prevents urination, or occurs along with discharge, sores, redness, or swelling.Possible Causes of Pain in the PenisPeyronie’s DiseasePeyronie’s disease starts when an inflammation causes a thin sheet of scar tissue, called plaque, to form along the upper or lower ridges of the shaft of the penis. Because the scar tissue forms next to the tissue that becomes hard during an erection, you may notice that your penis bends when it’s erect. Misconceptions in Sexual Health

Many people think that Masturbatory Habit ("Hasth Prayog"/"Mutti maar" etc) in males lead to sexual problems after marriage. Medically this concept is not correct and purely its a misconception.Scientifically this misconception spread from generation to generation making man shaky during adolescence and early adult life.Usually parents put this thought into growing young boy to discourage him into too much distracting sexuality by telling that its dangerous to loose sperm which is very important for manhood and loss of it would cause weakness, memory loss, body pains and on long run impotency etc.Thus, the boy in adolescence due to puberty with hormonal changes somehow start the masturbatory act but soon starts feeling guilty and scared. He cannot ask anything about it with parents/teachers or uncles and aunts. No proper sex education when its needed. Even if he stops this act forcible, he may be having "nocturnal emissions" (night wet dreams) which will make him feel bad within himself. He starts attributing many bodily symptoms like weakness, joint pains, paleness of face, no gain in body weight, poor attention and concentration with memory defects to this loss of Sperm. In fact whether used or not sperm will be produced daily in testes from puberty onwards. Its not that by masturbation it gets dried up in anyway. Many times before marriage many men get jittery wondering about possible development of Impotency etc due to his previous masturbatory habit.In fact, nothing happens like that. Boy while developing pubertal changes with penis growth, erections and sperm production and development of sexual desires etc are common and correct at that age. Doing masturbation in a technically correct way is nothing wrong. But overdoing the same/doing in wrong technique/using masturbation for sleep purpose etc are wrong. By doing so, in fact, night emissions may not happen at all. Some people try to cultivate "fast & secretive" type of masturbation due to lack privacy or fears etc which may cause conditioned early timing (habit) of releasing early which may lead to premature ejaculation after marriage.So safely and decisively it should be noted that its  NOTHING WRONG IN MASTURBATION IN CORRECT DOSE AND IN CORRECT FORMAT.Don't yield to such misconceptions and do not develop Psychological E.D. after marriage leading to unhappy and unsatisfactory conjugal life.

What is Erectile Dysfunction (ED)?A person having Erectile dysfunction (ED) is not able to achieve and/or maintain an erection of the penis sufficient for sexual activity. So, penis do not get hard at all or not enough for doing sex. ED is responsible for great deal of stress and anxiety as well as low self-confidence in men. Mostly its occasional, when you cannot get an erection and it is only temporary. But sometimes it occur repeatedly. There are lot of misconceptions about it in the mind of common man.Normally after emotional or physical stimulation there is an increased filling of blood in sponge like tissue of penis and decreased blood flow out of penis, so blood remain trapped in penis and it becomes hard and erect. Thus our ability to have erection is dependent of multiple factors like our emotions, hormones, nerves, blood vessels and cells of penis itself. A problem with anyof these elements can cause ED.It can occur at any age, but becomes more commonwith increasing age. In the older adult male, it is a major problem affecting more than 75% of men. Common Causes of ED:There are several factor's which causes bodily or mood changes & lead to ED. Foreskin Problems in Boys

Foreskin is the piece of skin that covers the top of the penis. Foreskin problems such as tightness, ballooning, redness are very common. It can affect boys of all ages. Inadequate or incorrect information can cause confusion and anxiety to both patients and parents. This article will help parents to get some understanding on common foreskin problems in boys.Tight Foreskin or PhimosisIn boys, tight foreskin, also called as phimosis is very common. Because of the tightness, Sometimes the foreskin balloons at the time of passing urine. At birth, the foreskin is normally tight (Figure 1). However, as the boys get older, fluid is produced from the undersurface of the foreskin and makes it freely mobile. By the time the boys are around 10 years of age, the foreskin is completely free and mobile. Beyond the age of 10 years,if foreskins are not freely mobile, treatment is usually recommended.Figure 1: Normal tight foreskin    Figure 2: Flower appearanceAlthough tight foreskin is very common in young boys,it can also be seen after infection or inflammation of the foreskin. In a normal tight foreskin (physiological phimosis), pulling the foreskin back will show a flower like pink inner foreskin at the tip, as shown in Figure 2. However, if infection is present (pathological phimosis) the foreskin tip is flat and flower appearance is not seen.Inflamed ForeskinInflammation of the tip of the penis and foreskin (balanitis) are usually caused by chemical irritation, bacterial or fungal infection. The tip of foreskin becomes red, swollen and painful (Figure 3). After some time the inflamed foreskin becomes tight – as shown in Figure 4. Antibiotic and antifungal creams can be applied, along with regular warm sitz (antiseptic) bath and it usually works well.Figure 3: Balanitis         Figure 4: Inflamed & tight foreskin                                                 BXOInfected or inflamed foreskin should always be watched carefully, as they can result in a tight scar formation (BXO -Balanitis xerotica obliterans) (Figure 5). Steroid creams applied over the foreskin may improve the tightness, but not in all patients. In many patients, the tightness comes back immediately after stopping the cream applications. Most patients need circumcision (Removing the foreskin) which will cure the patient.Very rarely, the infection from the tip and head of the penis can spread into the whole penis. The whole penis appears swollen and red (Figure 6)and this condition is called balanoposthitis. Oral antibiotics (tablets and syrup) are usually not enough. Antibiotics given via the veins (intravenous) will reduce the redness and swelling quickly.Figure 5:BXO         Figure 6:Balanoposthitis                                ParaphimosisSometimes boys forget to pull the foreskin forward (back to normal) after cleaning or passing urine. The foreskin becomes red,swollen and appears like a balloon around the head of the penis (Figure 7). This is called paraphimosis and it is an emergency requiring surgery. If noticed early, the foreskin can be gently pulled forward to normal position under anaesthesia, otherwise the foreskin may need to be removed – circumcision.Figure 7: Paraphimosis.   Figure 8:Foreskin cystCysts in the foreskin are very common. The most common cysts are white in color and move along with the foreskin(Figure 8). They are not painful at all but can cause lot of anxiety. The normal fluid produced from the foreskin and glans (smegma) is trapped under the foreskin and forms a cyst. These cysts resolve on its’ own and do not need any surgery.

Squeeze It - The Right Way!

And if your problem is Premature Ejaculation, let's get you a better solution.Introducing : BASILAR SQUEEZEThe standard squeeze involves pressing the penis near the glans which means interruption during penetrative sex. Now, who wants interruption? Right?Masters and Johnson devised a modified version of the squeeze to overcome this problem. They called it the BASILAR SQUEEZE. This involves the pressing of this penis at the point where it joins the scrotum. It can be squeezed with the correct movement.The squeeze is to be maintained for 4-8 second similar to the earlier method. They do admit that it should not be used as an alternative to the standard method, as it is not as effective.One of the major problems faced before entering into this therapy is that "other issues" have to be resolved. They are: dislike for each other (partners), anger, alcohol, tobacco, or drug abuse, infidelity, distrust, mental or physical torture, economic strain, etc. Medical conditions also have to be diagnosed and treated properly. In India, motivating the man to come and openly discuss these issues with his partner is very difficult.Important Considerations for this therapy:1. Time and repeated practice is essential to achieve ejaculatory control.2. ‘Refresher Course’ is required if gains made earlier are lost due to lack of time.3. Do not expect total control each time. Some failures are bound to occur.4. Change of position i.e. woman -on- top to side-by side to man-on-top is to be done slowly over a longer period of time and failures are to be expected in the initial stages.What are the alternatives?PHARMACOLOGICALThe first choice as mentioned before is behavioral but pharmacotherapy can be concurrently used for better results.ANAESTHETIC AGENTSExcessive sensitivity of the penis especially the glans has been thought to be one of the causes of premature ejaculation. Local anesthetics have been used with the intention of reducing sensory input from the penis thereby delaying ejaculation.RETARDED EJACULATIONRetarded ejaculation is defined as the man being able to ejaculate only after what seems to him to be an ‘excessive’ period of time. It is usually reported when the man requires ½ an hour or more of vigorous coital thrusting. It is interesting to note that most of these men do not seem to have any difficulties with erection but sometimes seems to hold them back and they are unable to trigger the ejaculatory reflex at the proper time.This is a very common symptom in men who are regular abusers of recreational drugs or alcohol. The usual story from the couple is that the woman becomes weary and uninterested once she has had her orgasm and the man becomes tried, exhausted and fatigue which is frustrating for the couple.Let's talk about TreatmentA three steps process is useful in treating delayed ejaculation:Teaching masturbating to the point of ejaculation as a solo activity. (focus on the "solo" part)Masturbation to the point of ejaculation with a partner. (focus on the "point of ejaculation" part)Finally, coital connection with ejaculation to be established. (focus on the "connection")This is easier said than done and the following aspects have to be considered:Allay fears, anxieties and myths about masturbating in both partners.Liberal use of lubricating jelly during masturbation.Mechanical means of stimulation with vibrators is useful stimulation which should initially be used for short periods.Encouragement of the partner to stimulate the penis with sufficient force and speed as many men use vigorous stimulation which the female may not understand.Use of fantasy to evoke psychological stimulus as many men ‘use’ only mechanical arousal methods.Those who cannot fantasize, could be advised to use erotic videos.Drugs like midodrine may help.
