Cellulite is often described an orange peel type dimpling of skin in the upper posterior thighs and buttocks. Although there is no associated morbidity or mortality, it is among the most common cosmetic complaints among female patients. Cellulite occurs when underlying fat deposits begin to push through layers of collagen fibres, or connective tissue (in the second layer of skin, often in the buttocks and thigh areas, but also on arms, stomach). Connective tissue can be weakened by hormones, lack of exercise and muscle tone, excess fat, and poor circulation.

One of the main reasons why it is more prevalent in women because of the subcutaneous fat is separated by thin fibrous tissue septae. The septae are oriented perpendicularly to the dermis which facilitates fat herniation into the dermis. In men crisscrossing fibrous septae divide fat chambers into smaller units more resistant to herniation.Oblique orientation of the connective tissues strands prevents herniation.

There are three treatable components of cellulite: You have to address the collagen; you have to reduce the fat, and you have to increase circulation.

But it depends on the grade of cellulite you have. There are four grades, ranging from zero to three:

GRADE ZERO is no visible cellulite. If you pinch the skin and see a cottage cheese–like texture—that is considered as grade one.

GRADE TWO is if cellulite is visible on the legs of someone standing.

GRADE THREE is if you see cellulite when you look in the mirror or lie down. I call grade three ‘terminal’ because it's very hard to treat.
