After 4 weeks of bi-weekly cholecalciferol supplementation from a starting level of 18, you've likely completed the loading phase. The standard approach is to recheck Vitamin D levels after 8-12 weeks of treatment to see where you stand. If levels are now in the sufficient range (above 30), you can shift to a maintenance dose of once weekly or once monthly. Without a recheck, it's hard to know exactly where your levels are now.
Next Steps
Get your Vitamin D level rechecked now (after 4 weeks of treatment), then consult your doctor to decide whether to continue the same dose, reduce to a maintenance dose, or switch to a monthly regimen. Please consult me directly on Practo for a detailed evaluation.

Stop the medicine and get your level check first and if it comes below the reference range, then start the medication again but it shouldn’t be taken biweekly.
Twice a week vitamin d is not usually prescribed, ideally should be taken once a week for atleast 8 weeks , followed by once a month for another 6 months
Cholecalciferol capsule once /week x 6 weeks followed by once/month 6 month
Also take Adequate calcium intake from diet or supplements
Recheck Vit D 25 OH, after 4-8 weeks start of the treatment
Based on this, you have an initial clinically deficient Vitamin D status. While your bi-weekly approach has introduced a steady amount of the nutrient, a standard protocol requires a structured alignment to completely replenish your body's deep bone reserves. However, for a proper diagnosis and to chart out your exact continuation path, we need to clinically cross-reference your upcoming serum values and check for any symptoms of muscle weakness or bone pain.
Next Steps
Cholecalciferol 60K (60,000 IU) once per week for eight weeks, followed by a maintenance dose of one per month for six months. Taking high-strength capsules twice a week without close medical supervision can rapidly increase accumulation, meaning a structured stabilization of your intake frequency is required to ensure optimal therapeutic efficacy safely.
