In pregnancy if TSH is 4 you need to keep a watch on it. Any symptoms should be noted.
Next Steps
Repeat free T4 and TSH after 2 weeks and get AntiTPO antibodies done. If it’s on higher side consult endocrinologist and discuss with your gynaecologist then follow the orders

A TSH of 4 in the first trimester is a little above what's ideally wanted in pregnancy, since pregnancy needs tighter thyroid control than usual. This doesn't mean anything is seriously wrong, but it often calls for a small dose of thyroid medicine such as levothyroxine, especially this early when your baby depends fully on your own thyroid hormone.
Next Steps
Get a repeat TSH along with free T4 and thyroid antibodies if these haven't been done, and discuss this with your obstetrician this week so treatment can start without delay if needed.
Health Tips
Please don't start or guess a dose on your own. The right starting dose depends on your exact TSH, weight and antibody status.
