Thick et on day 3 can mean two things, one is proper shedding hasnt started and second is polyp. If your flow was good during that time ,u should repeat a scan just as your menses finish to visualise a polyp.

Don't worry, have a repeat scan. If still polyp shows, get it removed by Hysteroscopy

Nothing serious but if there is a polyp it would  require surgical removal

On day 3 of your menses, if you have had a normal flow in ur periods then the ET should be thin - ideally less than 4 mm. Thick ET on day 3 could mean that either there is a polyp which has not shed. You should repeat your TVS on day 11 esp with 3 D USG for better diagnosis.
