This is a case of hypothyroidism. Does ur wife face menstrual irregularities? How many times has she gotten pregnant? Has she put on weight in last one yr? Since her TSH levels are elevated she has primary hypothyroidism but we don't know the cause . To rule out the differential pls get anti-TPO antibodies test and consult me for further treatment
