LHello Doctor, I've been experiencing sleep problems for over 3 months. Even if I go to bed around 11 PM, I often can't fall asleep until 2–3 AM, and sometimes as late as 5–7 AM. My sleep schedule is very irregular. I usually sleep 8–9 hours, but after several days of poor sleep, I sometimes sleep 12–18 hours. I have significant difficulty waking up and often need multiple alarms. Even after sleeping, I usually wake up feeling tired, and I occasionally feel sleepy during the day. I don't snore or have witnessed breathing pauses during sleep. I was diagnosed with migraine about 8 months ago, completed around 6 months of treatment, and have now stopped the medication as my migraines have improved. However, my sleep issues persist. I'd like to know if this could be a sleep disorder, what might be causing it, and whether I need further evaluation or treatment.

Possible causes include an irregular sleep schedule, excessive screen exposure at night, stress or anxiety, mood-related concerns, thyroid dysfunction, anemia, or medication effects. Your history of migraine and previous medications is also relevant.
Since your symptoms have persisted for over three months, consult a physician or sleep specialist. A detailed sleep history, medication review, and selected investigations such as CBC and TSH may be appropriate. A sleep study may be needed if another sleep disorder is suspected.
Next Steps
Maintain a consistent wake-up time daily.
Get natural sunlight in the morning and engage in regular daytime physical activity.
Avoid caffeine late in the day and reduce screen exposure before bedtime.
Avoid self-medicating with sleeping pills or melatonin.
Keep a sleep diary for two weeks, recording bedtime, sleep onset, waking time, and daytime sleepiness.
Health Tips
Please tell:
1.Do you feel sleepy before 2–3 AM, or do you feel completely awake?
2.Do you use your phone in bed or consume caffeine regularly?
3.Are you taking any current medications, or experiencing stress, anxiety, or low mood?
A pattern like this, falling asleep very late and sometimes needing 12 to 18 hours to feel rested, points more toward a circadian rhythm issue than a breathing-related sleep disorder, especially since you don't snore or have witnessed pauses.
Next Steps
Keep a simple sleep diary for 2 weeks noting bedtime, wake time and naps, and get a basic thyroid test done, since thyroid issues can disturb sleep this way too.
Health Tips
If you also notice low mood or appetite changes alongside this, mention it too. Please consult a physician or sleep specialist so this pattern gets evaluated properly.
