Take a deep slow breath in. Slowly let it out. Repeat 3 times. Then just breathe normally, not so deep. Focus on the inhale and exhale. When you notice your mind wondering, focus back on the breathe. It becomes easier overtime not to get distracted by your mind. There are aps on you tube you can download to help you with this.
Next Steps
consult Psychiatrist/ Psychologist
Health Tips
Do Yoga and meditation, Go for counselling session

Hello there. It isn't clear as to how long you have been feeling like this and if any particular event has caused this. If it's a one time event you could try to relax through simple breathing exercises. Also try to distract your self by going for a walk or listening to some music. If it has been going on for a while now you could consult a psychologist or psychiatrist who could evaluate you in detail and guide you further. Best wishes.
