I am having pain and probably a wound in anus opening. After passing motion, it pains and having itching. Whenever I am constipated, few days even blood comes through motion. I have been facing this issue after my c- section on and off (almost a year). If it touch I can feel a small wound at the edge of my anus. What could it be? Any serious issue?

This might be a condition called Anal Fissure usually caused by constipation and straining while passing stool.This needs proper treatment.
You need to consult with a surgeon or gastrosurgeon who will examine you and treat you properly.
It might be a fissure or sinus. Get it checked by a Gen. surg.
U can do Sitz bath for relief after passing stool, apply a metrogyl P oietnment.
Brisk walk and high fiber diet is recommended for healing.
Most likely you have a fissure.Constipation will trigger the pain and bleeding.Drink lots of water,use a high fibre diet, avoid constipation.You can apply anobliss cream locally and pilex ointment too.However to determine the exact cause, consult a surgeon in your area.
Take tab. Pilex two tablets twice daily. Apply oint anovate over the anus region. Eats foods rich in fibers and drink plenty of water. Taking the tablet won't interfere with breast feeding( if u feeding till now)

✔ Anti-fissure cream (Nitroglycerin 0.2%/Diltiazem 2%) to heal
If bleeding persists, consult a doctor. Not serious but needs care.
