Staying hydrated, resting, and taking medication such as acetaminophen or ibuprofen to reduce fever, with antibiotics prescribed if the fever is caused by a bacterial infection, under the guidance of a healthcare professional.
Since he is in a medical facility,be very assured. My advice would be to get a fever profile work up done. Are there any URT symptoms/soar throat, weakness?

If he is still having only fever with shivering then possibly he may be having malaria .
Next Steps
Go for complete blood count, peripheral smear for malaria parasite, urine routine/ microscopy, chest x ray
Drink plenty of water and maintain hydration.
Health Tips
Look for any rashes and if seen follow for platelet count.
Dnt worry, the fever will gradually come down. If all reports are normal, its a viral fever and the trend of viral fever is lasting for 7-10 days. Opportunistic infections will come down with antibiotics. Keep him hydrated and light food. He will be ok.

The fever may sometimes take longer to subside especially some viral fever. As he is already admitted in the hospital ,the treating physician will repeat those investigations if required to find the cause.
The first diagnosis is viral fever. It may last a week before the temperature starts coming down. The treatment is like for any fever.
Please share the CBC report if it has been done. Get his Dengue Antigen and Typhoid antibodies test done again. Get the urine culture done.
Some causes may be found now even though the tests were negative earlier.
After ruling out all kinds of fever, it's better to get blood culture done and urine culture based on that we can start the appropriate antibiotics instead of choosing some random antibiotics.
Through evaluation is needed.
Next Steps
fever profile
blood culture and sensitivity
urine culture and sensitivity
ESR and CRP
Would like to know his hemogram report.
Is there complain of abdominal pain?
Since it's 5th of fever now, it is advisable to go for NS1 antigen for dengue (Elisa) now and Chest Xray if not done.
Next Steps
S Widal if fever do not resolve after seven days
Abdominal sonography if not done
Routine and microscopic examination of urine
As per history your husband is suffering from either fever with rigour mostly with periodically you should admit one hospital first . Investigations again rule out. Smear study for malaria . Other routine investigations for fever evaluation.
Next Steps
Put him under strict monitoring and proper care
Health Tips
After obtaining all reports start suitable medication
Go for routine investigations mainly
1. CBC
2. Peripheral blood smear
3. Malarial parasite
4. Urine examination for routine and microscopic examination
To rule out Malaria and UTI
Drink plenty of fluids. Drinking fluids will improve heat loss from the skin and replace water lost through sweating. Water and clear broth are healthy choices. Infants under 6 months should only have breast milk or formula.
Rest. You need rest to recover, and activity can raise your body temperature.
Stay cool. If you aren't shivering, dress in light clothing, keep the room temperature cool, and sleep with only a sheet or light blanket.
Looks like viral infection
Some viral infections will take 7-14 days to get completely cured
Just keep him well hydrated if he doesnt have any heart related issues give him 3-4L of fluids per day
Dont.worry he will be fine in few days
Next Steps
consult internal medicine specialist if you noticw worsening of symptoms

**Opinion:**
I understand your anxiety regarding your husband's condition. Persistent fever despite normal test results and ongoing treatment can be concerning, especially when it recurs every 6 hours. The fact that he otherwise appears stable, with no joint pains or other significant symptoms, suggests that while his body is still fighting something, the underlying cause might need further investigation. This could be a less common infection, a reaction to medication, or another condition that hasn't yet been identified.
**Next Steps:**
1. **Comprehensive Evaluation:** It's important to conduct a more thorough examination and possibly run additional tests to identify the exact cause of the persistent fever. Advanced diagnostics, such as blood cultures, imaging studies, or specific viral panels, might be necessary.
2. **Medication Review and Adjustment:** The current treatment plan, including antibiotics, may need to be reviewed and adjusted. Sometimes, a change in medication can make a significant difference in how the body responds to treatment.
3. **Specialist Consultation:** In some cases, involving a specialist, such as an infectious disease expert, can provide new insights and lead to more targeted treatment.
4. **Ongoing Monitoring:** Continuous monitoring in the hospital is essential to ensure that any changes in his condition are promptly addressed. Staying hydrated and maintaining nutritional intake will support his recovery.
Given the complexity of the situation, I strongly encourage you to schedule a consultation with me. This will allow us to discuss your husband’s symptoms and treatment plan in detail, ensuring that every possible cause is thoroughly investigated and addressed. I am here to support you through this challenging time and to provide the best possible care for your husband.
