It’s mostly because of relapse. Correction can be done by wearing removable appliance for few months and then go for permanent retention. Composites or veneers can be other options but not that recommended. First choice is Ortho ( fixed or removable)

Dx : Relapse due to no permanent retention after orthodontic (braces) treatment.
Next Steps
Visit a orthodontist/dentist for braces followed by permanent retention options or you can try other options such as diastema closure with composite filling or veneers or crowns.

Based on your history, the gap may have reopened due to relapse after orthodontic treatment, not wearing retainers regularly, changes in your bite, or gum-related issues. Since you’ve already undergone braces twice, it’s important to identify the exact cause before planning treatment. I would recommend visiting an orthodontist for a clinical examination along with an OPG/X-ray and, if required, cephalometric records.
Next Steps
Depending on the cause, the gap can usually be corrected with: * Orthodontic retreatment (braces or clear aligners) if tooth movement is needed. * A fixed or removable retainer after treatment to prevent the gap from reopening. * If the gap is very small and the bite is stable, composite bonding may be an option in selected cases. * If gum disease or a high lip frenum is contributing to the gap, those issues should be treated first. From the photo, there appears to be spacing in the upper front teeth, but a proper examination is needed to determine the best treatment plan.
Health Tips
Could you please tell me: * Are you still wearing your retainers? If yes, how often? * Has the gap appeared gradually or suddenly? * Do you notice any bleeding gums, loose teeth, or tongue-thrusting habit?

Hello Mam . I understand your concern with recurrent gap formation in between upper central incisors. As per your given history of undergoing orthodontic treatment twice , i can analyse either your were not informed about Orthodontic Retainer or the retainer might have fractured ( fixed retainer) or irregular use in wearable retainer. I can also see tartar deposition in between upper front teeth.
Next Steps
Go for Scaling, retainer for further formation of gap or short term orthodontic treatment for the space recovery, or if you don’t want to go for orthodontic treatment again you can go for composite restoration or zirconia crown
