Leg pain with discoloration can occur due to different causes such as poor blood circulation (peripheral artery disease), venous problems (varicose veins/venous insufficiency), nerve issues (sciatica, neuropathy), or even vitamin/electrolyte deficiencies. Since your cardiac evaluation is fine, a vascular or neurologic cause should be considered.”
Next Steps
-Get a Doppler ultrasound of lower limb arteries and veins to check blood circulation. - Basic tests: Blood sugar, Vitamin B12, Vitamin D, Electrolytes. -If Doppler is normal but nerve pain persists- consult a Neurologist/Orthopedician for spine and nerve evaluation. -If circulation issue confirmed- consult a Vascular Surgeon.
Health Tips
Avoid prolonged standing or sitting; elevate legs when resting. Moderate walking, but stop if pain occurs. Use pneumatic compression socks/stockings to improve blood circulation. Maintain healthy weight and manage blood pressure, sugar, and cholesterol. Stop smoking if applicable.

Differential Diagnosis 1. Peripheral Arterial Disease (PAD) Classic: pain on walking (claudication), relieved by rest, foot discoloration, cold extremities. Needs vascular evaluation. 2. Chronic Venous Insufficiency / Varicose Veins Pain, swelling, and discoloration around ankles/heels. Worse on prolonged standing/walking. 3. Neuropathy (diabetic / compressive) Burning pain, sometimes discoloration secondary to poor circulation. 4. Dermatological causes Local skin conditions, but pain + walking-related worsening = vascular/neurological more likely. --- ✅ Advice to Patient Specialist to consult: Vascular Surgeon (first choice – to rule out arterial blockage). If vascular is normal → Orthopedic / Neurologist for nerve compression or neuropathy. For skin changes → Dermatologist (secondary). Investigations to suggest: Doppler Ultrasound (Arterial + Venous, lower limbs) ABI (Ankle-Brachial Index) Blood sugar, HbA1c (if diabetic suspected) Symptomatic Care until specialist visit: Avoid long walks/standing until evaluated Keep legs elevated while resting Avoid smoking (if applicable) Comfortable footwear Monitor for non-healing wounds or blackish discoloration → urgent consultation --- 📝 Sample Rx (for symptom relief, not definitive) Tab. Ecosprin 75 mg OD (if no contraindications, cardiologist already consulted → mild aortic sclerosis, so antiplatelet helps circulation) Tab. Pentoxifylline 400 mg TDS (if PAD suspected – improves blood flow; prescribe after Doppler confirmation ideally) Topical moisturiser for feet dryness/discoloration --- 🔔 Red Flags – Immediate ER Visit if: Sudden increase in leg pain Cold, pale, or bluish foot Non-healing ulcer / blackening skin

Provisional Diagnosis: Suspected Peripheral Arterial Disease / Venous insufficiency Investigations: • Doppler ultrasound of both lower limb arteries & veins • ABI (Ankle–Brachial Index) • CBC, Lipid profile, Blood sugar, Kidney function Treatment (general until specialist visit): • Tab. Ecosprin 75 mg OD (if no allergy/ulcer/bleeding risk – doctor to confirm) • Tab. Atorvastatin 10–20 mg HS (if cholesterol high or risk factors present) • Walking exercise (graded, as tolerated) • Leg elevation & compression stockings (if venous insufficiency suspected) • Lifestyle: stop smoking, control BP, sugar, cholesterol, weight ⸻ Note: Final treatment (angioplasty, bypass, vein procedure, or medicines) depends on Doppler results & vascular surgeon’s evaluation.

Next Steps
What You Can Do Right Now Check if your feet feel cold compared to hands, or if pulses in ankles are weak (this supports PAD). Avoid smoking (major risk for PAD). Keep feet elevated when resting if there is swelling/discoloration. Don’t ignore—poor circulation can worsen quickly if untreated.

Pain from knee to toe that worsens on walking along with discoloration of the feet can be due to vascular (blood circulation) issues or nerve/musculoskeletal causes. Since you already saw a cardiologist, the next step is to consult a Vascular Surgeon first (to rule out peripheral arterial disease or venous problems). If circulation is normal, then a Neurologist or Orthopedic specialist may be needed. What you should do: Get a Doppler ultrasound of leg vessels. Avoid smoking, control blood sugar/cholesterol if present. Don’t ignore foot discoloration — please see a vascular specialist soon. Bottom line: Start with a Vascular Surgeon for proper evaluation.
