Most commonly all of your symptoms can be attributed to the vitamin deficiency which is generally be due nutritional deficiency which means that you don't have enough intake of the given vitamins. And since you're a vegetarian, meeting this requirement through diet is very tough.
Next Steps
1) You should take cyanocobalamin injections and start on Oral Vit B12 supplementation 2) You should start a weekly oral supplementation of vit D3 along with a daily oral dose of vitD3+ Calcium For knowing the exact regimen of how to take the oral doses and injections feel free to book a consultation.
Health Tips
1) Take the supplements with milk as it increases the bioavailability of the drug 2) Avoid medicines used for gastritis as they further impair the VitB12 absorption. For any other query feel free to consult.

Your Vitamin B12 (140 pg/mL) and Vitamin D3 (7.6 ng/mL) levels are both significantly low and can explain your symptoms — body aches, weakness, tingling, and low BP. These deficiencies need to be evaluated and treated properly to avoid long-term complications. As you’re vegetarian, you should start vegetarian supplements for both vitamins: • Vitamin B12: Methylcobalamin 1500 mcg daily after food • Vitamin D3: Cholecalciferol 60,000 IU weekly for 8 weeks, then monthly Also include a balanced diet, sunlight exposure, and hydration. I strongly recommend a follow-up consultation to monitor your response and adjust treatment if needed. Early correction will lead to significant improvement in your symptoms. You may also need to get some other investigations to help treat your symptoms.

At these levels diet alone may not help. Please meet with a doctor who will prescribe you proper medicine for a while and also guide you about your diet and lifestyle changes to prevent a repeat of the problem You may also need to get some other investigations to help treat your symptoms. Only after a proper checkup should treatment be started so meet your doctor soon

Vegan source supplements: For Vitamin B12 : C Matilda forte one capsule once a day for one month For Vitamin D: Sunorb D3 (60kIU) sachets (1 sachet dissolved in 1 small cup cold milk) once a week for 8 weeks. However monitor your BP as low BP could be due to any other cause (dehydration/ cardiac etc); consult a physician if less than 90/60 mmHg.
Next Steps
Repeat Vitamin B12/ D3 after 3 months and follow up
Health Tips
Include dietary sources of B12 and D in daily diet. (Walnuts/ almonds/ green leafy vegetables/ fortified milk/ yoghurt, etc) Daily: 20 min exposure to early morning sunlight (7 am-8.30am)

Your symptoms—tingling, fatigue, and low BP—can indeed be linked to Vitamin B12 and D3 deficiency. Since your B12 is significantly low (140 pg/mL), this could be contributing to nerve-related symptoms. It’s good that you've identified it early. Since you are vegetarian, options for supplementation are available, but it’s best to consult a doctor directly to assess the severity and determine if injections or higher doses are needed initially. Please consult via Practo for proper evaluation and management.
Next Steps
detailed evaluation and personalized supplement plan. Neurological symptoms and low B12 may require prompt attention.
Health Tips
Helpful Tip: Ensure you get regular sun exposure for natural Vitamin D. Include B12-fortified foods in your diet. Word of Caution: Do not delay treatment for B12 deficiency, as prolonged low levels may lead to irreversible nerve damage and can cause dementia (memory loss) in the long run
