In a diabetic patient with obesity and history of renal stones, supplementation should be targeted rather than indiscriminate multivitamin use — first confirm levels (B12, Vit D, renal function). For B12 deficiency: oral methylcobalamin 1500 mcg daily or 1500 mcg weekly regimen (or IM if severe), and consider diabetic-friendly multis like Zincovit-DM / Diabeta Plus once daily after food. Avoid high vitamin C and excess calcium supplementation due to stone history; ensure adequate hydration.
Next Steps
Typical course: 3 months supplementation followed by reassessment and continuation based on clinical/lab response.
Health Tips
Advise lifestyle measures alongside — glycemic control, weight optimization, and periodic monitoring — as vitamins are supportive, not primary therapy.

Diabetes and B12 deficiency are common, especially in vegetarians. A B12 supplement (usually methylcobalamin) along with a balanced diabetic-friendly multivitamin can help. Avoid high-dose vitamin C or calcium supplements due to kidney stone history.
Next Steps
Dosage depends on blood levels, so proper evaluation is important. Please consult me for detailed guidance and personalized treatment.

You are a 48-year-old vegetarian man with: • Diabetes (or pre-diabetes) • Slight obesity • Vitamin B12 deficiency • History of kidney stones • History of UTIs This combination influences supplement choice: What we want to avoid/limit: • Excess calcium supplements (can worsen kidney stones) • High doses of vitamin C (oxalate risk) • Supplements with sugar or unnecessary fillers What you need: • Vitamin B12 (deficiency correction) • Vitamin D • Magnesium (often low and helpful for insulin sensitivity) • Omega-3 fatty acids • A balanced multivitamin/mineral that does not overload calcium Multivitamins are supportive but not a substitute for targeted corrections. Given your conditions, customization matters.
Next Steps
A. Vitamin B12 Preferred form: Methylcobalamin • Methylcobalamin 1500–2000 mcg daily (Or 1500 mcg 3–5 days/week) • If deficiency is severe → initially daily for 8–12 weeks, then rotate to maintenance. B. Vitamin D3 • 60,000 IU once weekly for 8 weeks, then • 2000 IU daily after levels normalize. C. Omega-3 (fish oil) Choose a product with EPA + DHA 1000–1500 mg daily: • Nordic Naturals Ultimate Omega • Viva Naturals Omega-3 • Coromega Omega-3 Squeeze (If vegetarian: choose algal oil DHA/EPA) D. Magnesium • Magnesium glycinate or citrate 200–300 mg at night Helpful for muscle cramps, insulin sensitivity, sleep. E. Multivitamin (balanced) Look for a diabetic-friendly, low-calcium, low-iron multivitamin: • Thorne Basic Nutrients 2/Day • Pure Encapsulations O.N.E. Multivitamin • Garden of Life mykind Organics Men’s Multi • GNC Mega Men Once Daily (low sugar fillers) These provide broad B-complex, antioxidants, and trace minerals without excess calcium or iron.
Health Tips
Cycle & duration • Vitamin B12: Start daily for 2–3 months → then 3–5x/week • Vitamin D3: Weekly for 8 weeks → then daily maintenance • Omega-3: Daily ongoing • Magnesium: Daily (preferably evening) • Multivitamin: Once daily with food Kidney stone consideration • Avoid additional calcium supplements unless deficiency proven. • Avoid high doses of vitamin C (>500 mg), which can increase oxalate. Blood sugar & safety • Take supplements after meals to reduce GI upset. • Monitor blood glucose regularly — some supplements can slightly affect insulin sensitivity. Interactions • Omega-3 can slightly thin blood — watch interactions if on aspirin or blood thinners. • Magnesium can cause loose stools if too high — adjust dose. ⸻ Given your specific health profile (diabetes, obesity, B12 deficiency, and past stones), you can absolutely benefit from structured supplementation — but the right mix matters more than random picks. If you want a personalized supplement and diet plan with precise doses based on your labs and medical history, I’d be happy to help you in a brief online consultation so we can tailor it safely and effectively.
