How can I get a discharge summary?

Discharge summary is handed over to patient/relative at the time of discharge.

I went for b12 low level dr Priyadarshini suggested b12 shots. I bought b12 injection and and syringe and gave to Male staff member .Male staff nurse asked me to wait outside. And he came with filled injection.

And then he called me to other room and injected.******* *** * *** **** ***** *** * *** ************* ******* *** ******* * ******* ****** ********

I went to all possible body checkup’s like hiv, hepatitis b, hepatitis c. Thank god all are negative.

But I have go through that pain.
