Noisy knees bring a lot of people to clinic, usually more worried than the finding deserves. The useful question is not whether your knee makes a sound. It is whether the sound comes with anything else.

Noise on its own is usually nothing

A painless click or crack when you stand up, climb stairs or straighten the knee is extremely common and present in a large proportion of people with entirely normal knees. It comes from gas bubbles in the joint fluid, from tendons flicking over bone, and from the kneecap tracking across the groove at the front of the thigh bone.

None of that is wear. A knee that clicks and does nothing else does not need an MRI.

When the sound does matter

Four combinations change the picture. Noise with pain in the same spot each time. Noise with swelling afterwards. Noise with the knee catching or locking so that you have to wiggle it free. Noise with the knee giving way under you.

Grinding rather than clicking is a different sound and carries different weight. A coarse, continuous grating felt through the hand on the kneecap while bending usually reflects cartilage surface loss rather than a passing mechanical sound.

What the different sounds tend to mean

Sharp single clicks on movement, usually benign. A soft rhythmic creak at the front of the knee on stairs, often patellofemoral, which responds well to loading work. Clunking with true locking, suggests a meniscal tear. Grating with pain and stiffness, suggests established arthritis.

These are patterns, not diagnoses. The examination distinguishes them far better than the description does.

After a knee replacement

A new knee that clicks is a separate matter and is common, particularly in the first year. Modern implants have a metal on plastic interface that can produce a soft click as the knee moves through its arc. It usually settles. A click that appears late, or comes with pain, instability or swelling, should be reviewed.
