Introduction
Wisdom teeth also known as third molars are the last to erupt and often the first to cause trouble. At Summirow Dental, we regularly encounter patients suffering from pain and discomfort caused by problematic wisdom teeth. Recently, a patient came to us with pain in both the upper right and left back teeth, and our diagnosis revealed that both tooth 18 and 28 were grossly decayed. To relieve the pain and prevent further oral health issues, extraction of both upper third molars was the best course of action.
Patient Complaint
The patient presented with:
- Pain on both sides of the upper jaw, especially while chewing
- Occasional swelling and discomfort in the back of the mouth
- Difficulty maintaining hygiene in the wisdom tooth region due to limited access
Clinical Examination & Diagnosis
A detailed clinical and radiographic evaluation revealed:
- Tooth 18 (upper right third molar) and tooth 28 (upper left third molar) were both grossly decayed
- Extensive destruction of tooth structure, making restoration impossible
- No signs of acute infection or abscess at the time, but the teeth posed a risk for future complications
Diagnosis:
Grossly decayed non restorable upper third molars (18 & 28)
Treatment Plan: Extraction of 18 and 28
Given the advanced stage of decay and the patient's discomfort, we advised extraction of both 18 and 28 under local anesthesia. This preventive and curative step would eliminate the source of pain and potential infection.
Procedure Overview
Local Anesthesia: Administered to numb the upper right and left posterior regions.
Hemostasis: Gauze was placed, and bleeding was controlled with pressure.
Post-Operative Instructions: The patient was given detailed aftercare guidelines to ensure smooth healing.
Post-Operative Care
The patient was advised:
- To avoid hot, spicy, and hard foods for 24–48 hours
- Not to rinse vigorously or use straws
- To take prescribed painkillers and antibiotics as directed
- To maintain oral hygiene without disturbing the extraction sites
