Introduction

Even mild sensitivity in a tooth can be the body’s early warning signal. At Summirow Dental, we encourage patients to take such symptoms seriously, as they often point to underlying issues like dental caries. Recently, a female patient visited us with mild sensitivity in her lower right back tooth (tooth 46). A thorough examination revealed deep dental caries but with timely treatment, we were able to restore her tooth with a deep Class II composite filling, preventing further damage and avoiding the need for root canal therapy.

Patient Complaint

The patient reported:

  • Mild, occasional sensitivity to cold food and drinks
  • No spontaneous or severe pain
  • General discomfort while chewing on the right side

Though the symptoms were subtle, she acted promptly by seeking professional care something we always recommend.

Clinical Examination & Diagnosis

A clinical and radiographic examination showed:

  • A deep carious lesion on the distal surface of tooth 46 (lower right first molar)
  • No signs of pulp exposure or irreversible pulpitis
  • The tooth was vital, and the surrounding tissue was healthy
  • Mild enamel loss and proximal cavity formation confirmed the need for a Class II restoration

Diagnosis:

Deep caries involving the dentin in tooth 46, with mild sensitivity but no pulpal involvement

Treatment Plan: Deep Class II Composite Filling with Proper Isolation

To preserve the natural tooth structure and prevent further progression, we opted for a deep Class II composite restoration using modern adhesive techniques and strict isolation.

Procedure Overview

  • Local Anesthesia: Administered for complete comfort.
  • Isolation: A rubber dam was placed to ensure a dry and clean working field, critical for adhesive success.
  • Cavity Preparation: All decayed tissue was removed carefully, maintaining as much healthy tooth structure as possible.
  • Liner/Base Application (if needed): In deep areas near the pulp, a protective liner was applied to prevent thermal or chemical irritation.
  • Etching, Bonding & Composite Placement: The cavity was etched and bonded with a high-quality dental adhesive.
  • nano-hybrid composite: was incrementally placed and cured to ensure a strong, durable, and aesthetic restoration.
  • Finishing & Polishing: The filling was contoured to restore natural anatomy and polished to a smooth finish for comfort and longevity.

Outcome

The patient experienced immediate relief from sensitivity and was pleased with the natural appearance of the filling. She appreciated the minimally invasive approach and was relieved to have avoided root canal treatment by acting early.

Conclusion

Deep caries doesn’t always mean root canal. When caught early, a well executed Class II composite restoration can save the tooth, relieve discomfort, and restore function. At Summirow Dental, we focus on preserving your natural teeth with the latest techniques and utmost care.
