When Tooth Pain Disrupts Daily Life
A healthy smile can quickly turn into discomfort when tooth pain strikes unexpectedly. A patient visited Summirow Dental with a chief complaint of severe pain in the lower right back tooth region. The pain was continuous and was affecting chewing and overall comfort. Like many patients, they were anxious and looking for quick relief.
Lower molars play a crucial role in chewing, and any infection in this region should never be ignored . Early diagnosis and timely treatment can save the natural tooth and prevent complications.
Clinical Examination & Diagnosis
On clinical examination, we noticed deep dental caries in the lower right first molar - tooth number 46. The tooth was tender, indicating pulpal involvement.
To confirm the diagnosis and evaluate the extent of infection, an IOPA (Intra Oral Periapical Radiograph) was taken .
Radiographic Findings:
- Deep carious lesion involving the pulp
- Periapical changes suggestive of pulpal infection
- Tooth structure otherwise salvageable
Diagnosis: Irreversible pulpitis with periapical involvement
Treatment Plan: Root Canal Treatment (RCT) in multiple visits
First Sitting - Root Canal Opening & Biomechanical Preparation
The patient was explained the entire procedure in detail to ease anxiety and build confidence . After obtaining consent, we proceeded with the first sitting of root canal treatment.
Procedure Highlights:
- Local anesthesia administered for a pain-free experience
- Root Canal Opening (RCO) done to access infected pulp
- Removal of infected and necrotic pulp tissue
- Biomechanical Preparation (BMP) performed using rotary and hand files
- Thorough irrigation using disinfecting solutions to eliminate bacteria
Proper irrigation is a critical step as it ensures complete decontamination of the canal system and improves long-term success of RCT.
Intracanal Medicament - Calcium Hydroxide Placement
After cleaning and shaping the canals, calcium hydroxide was placed inside the canals as an intracanal medicament .
Why Calcium Hydroxide?
- Kills residual bacteria
- Reduces inflammation
- Promotes periapical healing
- Ensures a sterile canal before final filling
The tooth was then sealed temporarily, and the patient was recalled after 5 days for the next appointment.
The patient reported significant pain relief after the first sitting itself - a reassuring sign of successful disinfection.
Second Visit - Obturation
After 5 days, the patient returned comfortably and pain-free .
On examination, there were:
- No symptoms
- No tenderness
- Healthy response from surrounding tissues
- We proceeded with obturation, which is the final and most important step of RCT.
Obturation Process:
- Canals dried thoroughly
- Filled with gutta-percha and sealer
- Ensured a tight, three-dimensional seal
- A well-obturated canal prevents reinfection and ensures the longevity of the treated tooth.Permanent filling was done to assure strength of tooth .
Patient Satisfaction & Outcome
The patient was extremely happy with the treatment outcome . The pain was completely relieved, and the patient could chew comfortably again.
Saving a natural tooth always brings satisfaction-not just to the patient but also to the treating dentist .
Why Timely Root Canal Treatment Matters
Delaying treatment can lead to:
- Severe infection
- Swelling and abscess
- Tooth loss
- Spread of infection to surrounding bone
RCT helps:
- Eliminate pain
- Save the natural tooth
- Restore function
- Prevent extractions
Final Restoration - What’s Next?
After RCT, placing a crown is strongly recommended for posterior teeth like molars to:
- Protect the tooth from fracture
- Restore chewing efficiency
- Increase longevity of the treated tooth
