Scarring is a natural part of the healing process after an injury. When skin is damaged, body creates new tissue to close the wound. This scar tissue, made mostly from the protein collagen,gives support and structure to the affected area. 

Scar tissue is thicker and more fibrous than normal skin.Scars may appear raised, sunken, discoloured, or thick,depending on the cause of the injury and how the skin heals. While some scars gradually fade over time, others may remain noticeable for years without treatment.

Common causes of scars

1. Acne

Acne scars are among the most common types of facial scars.Severe or inflamed acne can damage the skin tissue, leaving behind depressionsor dark marks.

Types of Scars:

1. Keloid Scars- Scars that grow bigger than the original wound. Instead of stopping when the skin is healed, the body keeps making collagen for months, years — after the injury, creating a raised scar. Occur in individuals with darker skin tones and those with a genetic predisposition to keloids.  Treatment options for keloid scars include corticosteroid injections, cryotherapy, and surgery.

2. Contracture scars -In burn cases.  These scars tighten the skin & restrictmovement. Can also affect underlying muscles, nerves. Treatment options for contracture scars include scar revision surgery, skin grafts, and physical therapy.

3. Hypertrophic scars - Also called raised scars,these scars may shrink over time but never completely flatten out & don'tgrow beyond the boundary of the injury. Treatment options for hypertrophic scars include silicone sheets, corticosteroid injections, and laser therapy.

4. Flat scars -These scars flatten and becomeless noticeable as they heal.

5. Atrophic scars- These scars appear as depressions in the skin and are often caused by acne or chickenpox. Also called ice pick scars. They occur when the skin fails to produce sufficient collagen during the healing process. Skin look uneven or wavy and may sometimes form deeper craters with defined edges. Treatment options for atrophic scars include dermal fillers, laser therapy, and chemical peels.

6. Stretch marks - Stretch marks are scars that form when the skin stretches quickly due to pregnancy, growth spurts, or fluctuations in weight. Stretching causes the collagen fibers to break down, leading to visible “stretch marks” onthe skin’s surface. Treatment options for stretch marks include topical creams,laser therapy, and microdermabrasion.

Scar Treatments 

There are several effective treatments to improve the appearance of scars.

1. Topical treatments

Certain medicated creams, silicone gels, or other topical treatments soften scars and improve their appearance over time. Products suchas alpha hydroxy acid, hydrocortisone, or retinoic acid may help lighten a scar, while gels made with silicone can shrink scars. Creams work by hydrating the skin and reducing inflammation, while silicone sheets and vitamin E oil can help improve the skin’s texture and colour.

2. Corticosteroid injections

Often used to treat raised scars, like keloids or hypertrophic scars. The steroids are injected directly into the scar to break down collagen fibers, help reduce inflammation,flatten the scar, and relieve discomfort. 

3.Laser therapy

 Laser treatment such as CO2 ablative, Erbium, Pulsed dye works by stimulating collagen production and improving skin texture. Reduces redness,flatten raised scars, and improve the appearance of acne scars.

4.Chemical peels

Chemical peels use specially formulated solutions to remove the top layer of damaged skin. This treatment helps reduce superficial scars,acne marks, and uneven skin tone.

5.Microneedling

Microneedling is a minimally invasive procedure that uses tiny needles to create controlled micro-injuries in the skin which activates the body’s natural healing mechanisms and promotes collagen production,improving skin texture and minimising the appearance of scars.

6.Dermal fillers

Dermal fillers such as collagen, fat, or hyaluronic acid can be used to treat depressed scars to add volume and lift the skin, making the surface look smoother and more even.

7. Bleomycin or fluorouracil. 

They can help flatten scars and relieve itching and pain.

8.Dermabrasion and microdermabrasion 

Removing the surface layers of the skin to blend in scar irregularities.

9. Cryosurgery 

Freezing the scar can reduce its size, pain, itchiness, hardness, and discoloration. This may be combined with steroid or 5-FU injections.

10. Scar revision Surgery

Such as skin grafts, skin flap, Z-plasty or W-plasty can reposition or reduce the size of a scar to make it less noticeable and more aesthetically pleasing. Not recommended for raised scars due to the risk of recurrence or more severe scarring.
