Also known as the road to health chart. It displays the child's physical growth and development.

A single weight record indicate the child's size at that moment, it does not give information whether the weight is increasing, decreasing or steady. Periodic weighing is necessary starting at birth and continued thereafter to note the progress of growth.
The child should be weighed :
  • 1st year- once a month
  • 2nd year- every 2 months
  • Upto 5 - 6 year - every 3 months

The weight should particularly be taken after any illness or when the child is sick.

For a baby of weight 2.5 - 4.0 Kgs at birth the weight gain should be -
  • 1 month - 2.75 - 4.25 Kgs. -51cm
  • 2 months - 3.5 - 5 Kgs -55.5cm
  • 3 months - 4 - 5.5 kgs. -61.1cm
  • 4 months - 4.5 - 6.5 Kgs. -63.4cm
  • 5 months - 5 - 7.25 Kgs (2 times the birth weight) -65.4cm
  • 6 months - 5.5 - 7.75 Kgs -67.8cm
  • 8 months - 6.5 - 9 Kgs -70.2cm
  • 9 months - 6.75 - 9.5 Kgs -72.3cm
  • 10 months - 7.25 - 9.75 Kgs 73.2cm
  • 11 months - 7.5 - 10 Kgs -74.9cm
  • 12 months - 7.5 - 10.25 Kgs -(3 times birth weight) -76.1cm
  • 1,1/2 years - 7.75 - 11.5 Kgs -79.1cm
  • 2 years - 8.5 - 12.5 Kgs -85.6cm
