Introduction:
A 34-year-old female patient presented with complaints of pain in the upper left back tooth region. The pain was localized to teeth #23 and #24, which had been previously treated with root canal therapy (RCT) and restored with crowns (CAP). Despite the initial treatment, the patient experienced recurring discomfort, which led her to seek further dental care.
Diagnosis:
Upon examination and radiographic analysis, it was found that the previously treated teeth #23 (left maxillary canine) and #24 (left maxillary first premolar) were showing signs of reinfection or complications related to the previous root canal treatment. These issues could have been caused by several factors, including:
- Incomplete removal of infected tissue during the initial RCT
- Leakage or failure of the crowns, allowing bacteria to enter the tooth
- New infections or cracks in the teeth, lead to the discomfort
The decision was made to perform a re-root canal treatment (Re-RCT) on both teeth to remove any remaining infected tissue, disinfect the canals, and ensure the teeth were thoroughly sealed. This would help alleviate the pain and prevent further complications such as abscess formation or bone loss.
Treatment Plan:
1. Re-Root Canal Treatment (Re-RCT):
- The first step was to carefully access the pulp chamber of each tooth (23 and 24), remove the old root canal filling material, and clean out any residual infection.
- The canals were then thoroughly disinfected, and new filling material was placed to seal the root canals effectively.
- Re-RCT aims to prevent reinfection and restore the health of the tooth structure.
2. Crown Placement:
- After the root canals were properly treated and healed, the next step was to place new crowns on both teeth. Crowns are essential in reinforcing teeth that have undergone root canal therapy, as they are more prone to fractures or wear.
- Custom-made crowns were selected for both teeth to ensure a proper fit, functionality, and aesthetic appeal. These crowns would restore the tooth's strength, improve the patient's bite, and enhance the appearance of the affected area.
Outcome and Prognosis:
Once the Re-RCT and crown placement are completed, the patient can expect significant relief from the pain in the upper left back tooth region. The teeth will be restored to full function, allowing the patient to eat, speak, and smile comfortably once again. Regular follow-up visits will be scheduled to ensure that the teeth remain healthy and functional.
In this case, the combination of re-root canal treatment followed by crown placement offers a comprehensive solution to restore both the health and aesthetics of the patient's teeth. With proper care, these teeth should remain functional for many years to come, providing long-term relief from pain and preserving the integrity of the patient's smile.
