What does this circle one show on x-ray?
I have severe pain in tooth 2ndmolar GUM.Had x-ray&dentist circled this x-ray.Didn't say what that is. This is second opinion as first Endo said RCT. 2nd dentist said extraction.Do I need to do root canals or extraction?I had bad tartar buildup on that tooth as shown in pic(took before SRP).I had deep cleaning in May.Went back to dentist for gum pain in May&they did deep cleaning.Went back in June (still pain),he said that tooth got tartar again within a month&scraped it out.Gum is so painful throbbing for pressing with fingers,touching,probing.Probing depth was 4 before deep clean.He said it's inflammation&need RCT.Second dentist said ext. Does this circled one in the x-ray 47 is infection?Do I need to do ext as soon as possible?How soon should I do Ext or RCT on this?I am currently out of India and planning to get treated in India in 2 months.Is 2 months too much time,will infection spread?severe gum pain. CBCT taken,Dr said inflammation seen on scan. Does cbct show it? Gum infect?

Small one could be carious tooth (appears radiolucent compared to the rest of the tooth). Needs to be physically examined. Big one could be a bony spicule after extraction. If so, soft tissue overlying the area will be tender and sore to touch. There's absolutely no indication for extraction of 47. If the small circle turns out to be caries, that could give you immense pain but the radiolucency is NOT very close to the pulp to present with such symptoms. However, since you are giving history of pain in second molar only, then you can give it a benefit of doubt. I don't see any urgency for any treatment. You can come back to India and get yourself treated. Meanwhile manage with painkillers. But before even jumping on to RCT please get your ipsilateral Ear, TMJ, and opposing quadrant evaluated and rule out a vertical fracture in second molar that's troubling you. Only after ruling out these all possibilities you must undergo RCT. Proper examination by a qualified dentist, preferably an Endodontist or Oral medicine expert, is essential for you.
Next Steps
Ask your dentist to check for a catch point on distal aspect of second molar before proceeding onto RCT.

I don't clearly see any indication for RCT or even extraction.Please be in followup and maintain oral hygeine this will suffice. However a clinical examination would give a better insight for further treatment needs.
