For the past two months, my father 79 has been taking different medicines to get relief from a “block” or “jam” feeling between his upper abdomen and chest. The medicines he has tried are:
1. Itovyom/Ganaton
2. Bloatpill
3. Voltapraz 20
4. Omez DSR
5. Sucral O
6. Razo L
7. Acogut ( Lupin)
However, none of these have really helped him. His endoscopy report mentions “gastric erythema and erosion,” but nothing about a hiatal hernia.
His ECHO heart test and BNP test are normal.
Could this still be a case of a small hiatal hernia?
Should we go for a Barium Swallow Test to check for it, since the endoscopy didn’t mention it?
I’m attaching a picture marked with a red line to show the exact area where my father feels this block/jam sensation — between his upper abdomen and lower chest.
Please advise.

No need of barium swallow. He should take small frequent meals, avoid spicy, fried food, take more of fresh vegetables, fruits, curd. Avoid eating late in the evening. Avoid excess of tea, coffee..Maintain normal, healthy weight.
Sleep with head end slightly elevated.
Tab omeprazole 20mg one morning and one evening before meals.
No need of barium swallow or nothing
Start cap rablac dsr twice daily before meal
Syp dulcolax 2 tsf in one glass of water at bed time 10 days
Probiotics
Plenty of fluid intake
Avoid use of pillow
Start walk and yoga for 30 minute
Avoid oily food and acidic things

I understand your concern. The âblock or jamâ feeling between the upper abdomen and chest can be quite distressing and may occur due to issues related to the stomach or food pipe, even when heart tests are normal.
1. Advices
Take meals in smaller portions and avoid lying down for at least two to three hours after food.
Raise the head end of the bed slightly while sleeping and avoid spicy or heavy meals at night.
Continue one single acid medicine such as Razo L before breakfast daily for at least two weeks without changing brands frequently.
2. What to Monitor
Whether the sensation worsens after food or while lying flat.
Any new symptom such as pain radiating to the back or unexplained vomiting.
3. Management
Since endoscopy showed gastric redness and erosion but no hiatal hernia, yet the symptom persists, it is still possible that a small sliding hernia was missed. Such small hernias may not always appear during endoscopy.
You may go for a Barium Swallow Test, which gives a clearer picture of small or intermittent hernias. If that is normal and the discomfort continues, an esophageal manometry test can be considered later.
Till then continue the acid medicine regularly and follow lifestyle precautions.

If there is repeated vomiting or black colored stools.
