1. Inguinal Hernia – Part of the intestine pushes into the groin or scrotum. Common in older adults and worsens with lifting.
2. Varicocele – Enlarged veins in the scrotum, more common on the left side.
3. Epididymitis or Orchitis – Infections or inflammation of the testicle or epididymis (less likely without fever or urinary symptoms).
4. Hydrocele – Fluid collection around the testicle, usually painless but can cause heaviness and discomfort.
• Your father should see a general surgeon or urologist for a physical examination and scrotal ultrasound.
• Avoid lifting heavy objects until properly diagnosed.
• A hernia, if present, may need surgery, especially if it’s causing chronic pain.
Temporary Relief:
1. Tab Paracetamol 650mg — 1 - 0 - 1
2. Scrotal support or tight underwear: May reduce discomfort
Next Steps
Consult a general surgeon

After carefully reviewing the complaints and considering he is a father of 19yr old. This could probably be a case of Inguinal Hernia or A Hydrocele which aggravates on doing some physical labor. This pain can only be subsided on temporary basis with OTC NSAIDS but which is only temporary. He needs to be evaluated first clinically and the based on radiology scan a definite diagnosis can be delivered resulting in an appropriate treatment and management. Please feel free to connect for further evaluation and management steps.
