Unresolved chest pain and palpitations
In July last year, I had sudden palpitations (pulse 150). ER diagnosed sinus tachycardia and hypertension. ECG and Trop T were negative ER sent me home and prescribed me to take lonazep 0.5 mg but i never took it I was sacred for some reason I don't know why. Since then, I often get sharp left-sided chest pain, dizziness, palpitations, and occasional breathlessness. Over the year, I did ~10 ECGs during episodes (all normal), 2D Echo normal. Lipids: LDL 113, HDL 49, triglycerides 106. Thyroid (TSH, FT4), glucose (HbA1c, estimated average glucose), CBC all normal. BP is controlled on medication. Two cardiologists and three MD physicians said I’m fine. I have BMI ~42. I also experience occasional GERD symptoms. Despite reassurance, symptoms persist. Could these be related to hypertension, obesity, or another cause?

Age is not mentioned but anyway major issue is obesity, palpitations, tachycardia left side chest pain , check holter , 2 d echo , thyroid profile , try loosing weight if can’t make , consult dietitian, gastroenterologist. You can consider for bariatric surgery yes GERD is due to obesity and I think you snore during the night time when I consider your BMI.
Next Steps
First consult cardiologist, and gastroenterologist, endocrinologist dietitian, self motivation and don’t loose hope kill your procrastination

Uncontrolled BP, GERD could cause atypical chest pain. Obesity could contribute indirectly.Weight reduction could significantly reduce your symptoms.

Hi I am a psychiatrist and I think you maybe severly preoccupied with your symptoms. I am sure it must be causing a lot of distress to you. The real reason for these physical symptoms lie in our mental state which may be disturbed due to a completely unknown or unrelated stressor. You need to get to the root cause of the stressor.
