Triple Marker Positive/ 1:182 Trisomy21
Hello Mam I am a 33yr old female. This is my second pregnancy. My NT scan was normal but triple marker test was positive with 1:182 ratio positive screening for Trisomy 21. The matter of concern for me is that the value of unconjugated estriol (uE3) is extremely low which is 0.13 ng/mL at 14 weeks 6days pregnancy also my Triple Marker was done twice by the lab to rule out any discrepancy. Doctor suggested NIPT. But we want to directly go for abortion as the risk is high and estriol value is extremely low which also depict it could also be placental issue. Kindly someone suggest something. We are extremely worried.

Hi First of all triple test screening is obsolete. It is not at all advised by our Fetal medicine people. Coming to values, we consider Mom value not the exact value of the harmone. Dnt go with the Google.

NIPT is also a screening test with 99.8% prediction rate but it is still not confirmatory. Invasive procedures like CVS and Amniocentesis are also available which are Diagnostic and are quite safe in experienced hands.
Next Steps
Think wise, if affordability is not an issue you can go for the procedures else you can opt not to continue the pregnancy.

Triple marker is a screening test, not diagnostic. A risk of 1:182 means chances are still higher that the baby is normal. Isolated low uE3 can be seen due to lab variation, dating issues, or placental factors and does not confirm Down syndrome by itself, especially with a normal NT scan. It is strongly advised to do NIPT or a diagnostic test (CVS/amniocentesis) before taking an irreversible decision like abortion. Please consult a fetal medicine specialist for proper counseling.
