Hi,The extra tooth in the middle is called mesiodens for its extraction xray has to be taken to locate its exact position to make extraction easy.Extraction is not painful if proper local anaesthesia is given.If the tooth is infected extraction should be done under antibiotic cover .But before extraction treatment for gap closure should be planned.
