As the old filling has gone the decay has reached the pulp tissue that is the nerve area .
And so there is swelling .
Treatment will be pus drainage . Followed by root canal and then cap placing.

Hi... As per your history it looks you have swelling due to infection. You can visit my clinic in Pune for free consultation. Once fillings are out you get sensitivity also. Good composite resin fillings if done correctly can last you for many yrs. The painful tooth will need urgent care. You can apply ice from outside of cheek for little pain relief.
Swelling can possibly be because of exposure of the tooth nerve and/or involved gum infection. The fillings that have been out seems one of the reason for teeth sensitivity. The teeth need to be examined so as to assess them for either fillings and/or root canal treatment
