Severe stomach pain in the midnight during the sleep. sure whether it’s gastric related or related to her period, she had a vada pav with chillies in the evening and hasn’t eaten anything since then. she stays in a hostel where there isn’t much access to proper food. what would you recommend... should she consult a general physician or a gynaecologist?

A GP can examine the abdomen and determine whether this looks gastric, intestinal, urinary, or pelvic. If the examination suggests a gynaecological cause, she can then be referred to a gynaecologist. Period pain typically occurs just before or around menstruation and is usually crampy lower abdominal pain. However, if the pain is genuinely severe right now, especially if it is persistent or worsening, she should go to an emergency department/24-hour hospital rather than waiting for a routine consultation
Health Tips
For the patient right now If the pain is mild/moderate and already settling: * Sip water/ORS slowly. * Don’t continue fasting; once nausea settles, have something bland/light such as banana, toast, rice/khichdi, curd or biscuits. * Avoid chilli, fried food, coffee and NSAIDs such as ibuprofen/naproxen until the cause is clearer. * A warm heating pad can help if it is typical menstrual cramping.

In my view, the spicy meal on an empty stomach makes acidity or indigestion more likely, though period-related pain is also possible. Severe, persistent or recurrent pain needs examination. Please take light food and fluids, avoid spicy/oily meals, and monitor the symptoms. Seek urgent care if the pain worsens or there is repeated vomiting, fever, fainting or black stools.

Your stomach pain after a spicy meal on an empty stomach points more toward acidity than anything else. If the pain also lines up with your period dates, a hormonal link is possible too.
Next Steps
Eat on time and avoid very spicy food for the next few days. Keep track of whether the pain returns around your period. If it repeats or does not settle within a day, please see a gynaecologist or general physician.
Health Tips
Please seek urgent care if the pain is severe or comes with vomiting or fever.
