I am experiencing severe sleep disturbance with a highly irregular sleep pattern. I currently sleep from 7 AM–11 AM and 2 PM–5 PM, yet have persistent daytime sleepiness, low energy, and impaired daily functioning.
I have had ~3 recent episodes of sleep paralysis involving inability to move, attempted awakening without success, and associated breathing difficulty.
Past history of anxiety and depression; was on psychiatric medication for 2–3 years with initial improvement but discontinued 2 years ago due to side effects.
I have attempted lifestyle changes (e.g., walking) and melatonin; however, melatonin results in sleep onset followed by awakening after ~2 hours.
Kindly advise on possible causes and which doctor should I consult (neurology/sleep medicine vs psychiatry or MD).

Your sleep pattern is like your brain is in a state of a jet lag. By sleeping in two short naps during the day you aren't getting the deep sleep your brain needs. Sleep Paralysis happens when your brain wakes up before your body Itâs a side effect of very irregular sleep schedules and exhaustion. This combination of daytime sleepiness and paralysis is common in Narcolepsy, Sleep Apnea or a Circadian Rhythm Disorder.
Next Steps
consult a Sleep Medicine Specialist a Neurologist or a Pulmonologist. They will order a Sleep Study to see exactly whatâs happening while you drift off.
