Altered adt/alt ratio with the normal liver enzymes is not an issue. His symptoms could be due to gastric issues or hepatobiliary issues. He could repeat the ultrasound scan abdomen if it's been more than a year

Bilirubin direct and indirect are on the higher normal side ,same is with OT/PT ratio. Nothing alarming. Grade one fatty liver can be reversed with dietary changes by avoiding fried food , light exercises and avoiding alcohol. Regarding polyp in Gall bladder which might be the cause of abdominal pain and pale stools ,you should consult a surgeon.

Hello Madam, I’ve reviewed your husband’s liver function test (LFT) report. The results are largely within normal limits. The SGOT/SGPT ratio (1.47) is slightly above the upper limit but still not worrisome by itself, especially since both enzyme values (SGOT 21.3, SGPT 14.5 U/L) are well within normal ranges. The bilirubin levels are only marginally elevated (total 1.04 mg/dL, indirect 0.83 mg/dL), which can occur due to mild fatty liver, fasting, dehydration, or Gilbert’s syndrome (a harmless condition). However, since he has fatty liver and small gallbladder polyps, the right upper abdominal discomfort after meals could be due to: • Fatty liver–related dyspepsia or mild hepatic congestion, or • Gallbladder polyps or stones irritating the gallbladder. Loose, pale stools can indicate mild bile flow disturbance. Recommended next steps: 1. Ultrasound abdomen (if not repeated in the last 6 months) — to check fatty liver grade and gallbladder status. 2. Avoid oily and heavy meals, alcohol, and fried food. 3. Maintain adequate hydration and regular exercise. 4. Liver-protective diet: include high-fiber foods, green leafy vegetables, coffee (if no contraindication), and avoid unnecessary supplements. 5. Consult a gastroenterologist if pain or pale stools persist — to evaluate bile flow and polyps further. At present, no urgent issue is seen in his LFT, but monitoring and lifestyle correction are advised.

SGOT/SGPT ratio 1.47 + pale stools = urgent evaluation needed. Ratio >1 suggests alcoholic liver disease pattern. Combined with fatty liver, GB polyps, and new symptoms (pain after meals, pale stools) indicates possible bile duct or pancreas involvement. Urgent tests: Repeat LFT + GGT/ALP, Lipase/Amylase, Ultrasound abdomen Action: See gastroenterologist THIS WEEK. Stop alcohol completely. Strict low-fat diet. Pale stools are red flag - don't delay.
Health Tips
SGOT/SGPT ratio 1.47 with new symptoms needs attention. LFT Analysis: - Ratio 1.47 suggests ALCOHOLIC liver disease pattern - Normal ratio is 0.8-1.0 - Ratio >1 suggests alcohol or cirrhosis - Fatty liver grade 1 present New Symptoms Are Concerning: - Right upper abdomen pain after meals - Pale loose motions = possible bile/fat malabsorption - Could indicate: 1. GB polyps growing (need monitoring) 2. Bile duct issues 3. Pancreatic involvement 4. Worsening fatty liver Urgent Tests Needed: 1. Repeat LFT with GGT, ALP 2. Lipase/Amylase (pancreas) 3. Ultrasound abdomen (check GB polyps size) 4. Stool for fat globules Treatment: 1. Stop alcohol completely if consuming 2. Liver protection: Ursodeoxycholic acid 3. Low-fat diet strictly 4. Digestive enzymes before meals Red Flags: - Polyps >1cm need surgery - Pale stools = serious - Worsening pain = urgent evaluation Recommendation: See gastroenterologist urgently - don't delay.

Hello, I am Dr. Romain Rajan, General Physician. Your case has been allotted to me. I understand your concern. The symptoms you described such as right upper abdominal pain after meals, pale loose stools and existing fatty liver with gallbladder polyps suggest that the liver and gallbladder are mildly inflamed or sluggish. The SGOT and SGPT ratio being slightly altered is common in such conditions and usually reflects fatty liver related enzyme changes rather than any serious damage. ✅ Advices Avoid oily, fried and spicy food completely for the next four to six weeks. Eat small frequent meals and stop eating immediately once full. Avoid alcohol, painkillers and any unnecessary supplements. What to Monitor Worsening pain, nausea or vomiting after meals. Yellowing of eyes or dark urine which can indicate bile flow disturbance. Management Start with • Tablet Pan D once daily before breakfast for 10 days to reduce acidity and nausea. • Syrup Liv 52 one teaspoon twice daily after meals for liver support. • Continue a high fibre diet with fruits like papaya, apple and plenty of water. You can also apply a warm compress on the right side for relief if the pain is dull. When to Consult a Doctor Immediately If pain becomes severe or persistent even without eating. If stools turn very pale or clay coloured. If fever, vomiting or yellowing of skin develops. Since gallbladder polyps can occasionally change over time, a repeat ultrasound every 6 to 8 months is advised to ensure there is no growth in size. Every patient’s liver condition differs. For accurate guidance and lasting relief, it is important to review the full LFT report and ultrasound details together. 👉 You can reach out to me directly for this case and also connect with me in future for any medical consultation. I am available on Practo or on WhatsApp at 85271646 seven seven for a one on one consultation where you will receive A personalized liver recovery plan Safe diet and medicine guidance Quick follow ups to track enzyme improvement Take care and wishing your husband good health. Best regards, Dr. Romain Rajan General Physician
