Since you have previously completed rabies vaccination, Rabies Immunoglobulin (RIG) is NOT required, even though there was a small amount of bleeding. For re-exposure in a vaccinated person, only rabies vaccine booster doses are needed, as advised by your doctor. RIG is given only to people who were never vaccinated earlier. Please ensure: • Proper wound washing with soap and water. • Complete the booster doses on time. Your current treatment is correct and sufficient. ⸻ Dr. Shameerh Basha, MBBS, CPCDM General Physician & Diabetologist Nava Karnataka Multi-Speciality Hospital, Ballari

No, you do not need immunoglobulin (RIG or ARS).You are fully in the "previously vaccinated" category according to WHO, CDC, and Indian guidelines (NCDC/NAPRE): You had 2 booster doses in May 2025. Then a full 4 intradermal dose series, with the last one on November 22, 2025. Today's bite happened in January 2026 — so your last vaccination was only about 2 months ago. This means your immunity is very recent and active For previously vaccinated people (even with boosters or recent full courses): Standard PEP after a dog bite (Category II or III, with minor bleeding) is only wound washing + 2 booster doses of vaccine: one on day 0 (the day of the bite) and one on Day 3

As per the standard guidelines, after the last dose of anti rabies vaccine if a person has another dog bite within next 3 months there is no need of anti rabies vaccine during this time. Once you have received complete schedule of antirabies vaccination anytime of your life there is no role of immunoglobulins. Immunoglobulin is given to a person who had no previous exposure to anti rabies vaccine in life time and the person is having category III dog bite where the risk of contracting rabies is very high hence the wound area is infiltrated with immunoglobulins. It provides instant immunity while anti rabies vaccine takes more than one week to start develop anti bodies in the blood. Hope that answers your all queries. Category III exposure (transdermal bite/scratch, licks on broken skin, mucosal exposure) of dog bite : • If NOT previously vaccinated → Rabies Immunoglobulin (RIG) + vaccine is required. • If previously vaccinated → RIG is NOT required, because: As you’ve already mentioned that you have received 4 intradermal dose of the vaccine . Which I’m assuming you received a Full post-exposure vaccination routine earlier (with cell-culture vaccines like PCECV/HDCV you have memory antibodies from the previous vaccines . RIG is only meant to provide passive immunity until antibodies develop, which is unnecessary in vaccinated individuals as you.
Next Steps
No need for RIG. You only need 2 booster doses on day 0 and day 3

Since you have already taken the vaccine, and the current bite is category 3 bite, you don't need RIg unless you have direct nerve exposure.
Next Steps
I can guide you well with some more history and clinical details.

You have already completed full anti-rabies vaccination earlier (4 intradermal doses + booster doses). In such previously vaccinated persons, the management after a fresh dog bite is different. Rabies Immunoglobulin (RIG) is NOT required if: The person has previously completed full rabies vaccination Booster doses were taken in the past Bite is minor or even Category III According to WHO and National Rabies Guidelines: Previously vaccinated individuals should NOT receive rabies immunoglobulin, because their body already has immune memory. Only 2 booster doses of rabies vaccine (Day 0 and Day 3) are sufficient.
Next Steps
Immunoglobulin is NOT needed Wound washing with soap and running water for 15 minutes is very important Booster vaccination as advised is adequate protection
Health Tips
If you still have anxiety, deep wound, or the dog’s vaccination status is unknown, clinical examination is important before changing treatment. For detailed guidance and proper documentation, you can take a paid consultation with me so I can guide you accurately as per your case.

Since you have already completed a full rabies vaccination course recently, you are considered previously vaccinated. In such cases, even if there is minor bleeding, rabies immunoglobulin is NOT required. Only booster doses are needed.
Next Steps
Complete the booster doses as advised (usually day 0 and day 3) Keep the wound clean with soap and water.
Health Tips
You’re already protected, so don’t panic. Extra vaccine doses won’t harm you, but RIG is not needed in your situation.

You are already well protected because you completed a full course of 4 intradermal rabies vaccine doses (last one on 22 Nov 2025 — very recent, just about 2 months ago). This counts as previously vaccinated under WHO and CDC guidelines. Today’s dog bite with little blood is a Category III exposure (transdermal bite with bleeding). For previously vaccinated people like you, the standard PEP is only 2 booster doses of vaccine (on day 0 and day 3) — NO rabies immunoglobulin (RIG/HRIG) is needed. RIG is only for people who have never been vaccinated (or very old/incomplete vaccination) to give immediate antibodies. Since your full schedule was recent and complete, your immune system will quickly produce strong antibodies (anamnestic response) from the boosters — adding RIG could even interfere with this natural boost. The doctor correctly gave you 4 intradermal doses (likely starting today as day 0), but according to current guidelines (CDC July 2025, WHO), you only need 2 boosters (day 0 + day 3). The extra doses won’t harm you (rabies vaccine is very safe), but they’re not necessary.
Next Steps
• You do NOT need immunoglobulin (RIG) — it’s not recommended and usually not given to fully vaccinated people. • Complete at least the 2 boosters: Finish the one started today (day 0) and get the second on day 3 (in 3 days). If the doctor already planned 4 doses, you can safely follow it (no harm), but ask them why — perhaps local protocol differs slightly. • Immediately: • Wash the wound well with soap and water for 15 minutes (already done hopefully). • Monitor the wound for infection (redness, swelling, pus, fever) — see doctor if worse. • If you’re worried about your immunity (e.g., due to any health issue), ask doctor for a rabies antibody titer test in 2-4 weeks (should be high). • No need for more unless a new bite happens.
Health Tips
• Always clean any animal bite/scratch right away with soap/water — this reduces risk a lot. • Avoid stray dogs; report the dog if possible (for observation). • Rabies vaccine is extremely effective — with your recent full course + boosters, you’re very safe. • Stay calm — rabies is preventable with what you’ve done. For more personalized reassurance (based on exact vaccine type, bite location, your health), please consult with me online — I can help explain or guide what to ask your doctor. You’re doing the right thing — stay safe!

For a re-exposure in a previously vaccinated person, RIG is NOT indicated, even if there is bleeding. Management is booster vaccination only.

As per your case , there is no need to take immunoglobulin but you need to follow these instructions 1)Do proper wound washing with and running water for almost 15 minutes. 2)You need to take only 2 doses of booster rabies vaccine ,which are usually taken on Day 0 and Day 3(since you are already vaccinated).
