In the intradermal (ID) route the volume is always 0.1 ml per site - it never changes with brand, with day, or with wound severity. Only the NUMBER of sites changes. For someone taking rabies vaccine for the first time (no previous complete course), the Updated Thai Red Cross / IPC intradermal regimen is 2 sites on Day 0 (0.1 ml into the skin over each deltoid, so 0.2 ml in total that day), then 2 sites again on Day 3 and 2 sites on Day 7. For a BOOSTER - that is, a fresh exposure in someone who already completed a proper pre-exposure or post-exposure course earlier - it is only 1 site of 0.1 ml on Day 0 and 1 site of 0.1 ml on Day 3. Nothing on Day 7 or Day 28, and no rabies immunoglobulin at all, regardless of how deep the new bite is, because your memory cells respond within 48-72 hours. So, answering your Day 0 question directly: first-ever course = 2 x 0.1 ml on Day 0; booster = 1 x 0.1 ml on Day 0. The other point worth knowing is that the ID route is dose-sparing and equally effective, but only if the technique is right - the needle must stay within the skin layers and raise a small pale bleb. A dose that disappears without a bleb has gone subcutaneous and does not count.

Day 3 means 3 days after Day 0; if you miss it, take it as soon as possible and count subsequent days from the original schedule rather than restarting. 5) Wash any fresh wound with soap under running water for 15 minutes, apply povidone-iodine, leave it unsutured if possible, and check your tetanus status. 6) If you are a first-timer with a Category III exposure (bleeding bite, mucosal contact, licks on broken skin), insist on rabies immunoglobulin infiltrated around the wound on Day 0 along with the vaccine. 7) Bring your card to a doctor if any dose was given intramuscularly by mistake - the remaining schedule then needs to be planned, not simply continued as ID.
