Hello sir . I understand your concern with confusion of treatment plan. To start with the tooth is deeply caries and is impacted. The xray you have attached is a 2D xray . As the tooth shows close proximation with the nerve Chanal passing just below is a bit concerning.
However the tooth needs surgical extraction. It is a very regularly done minor surgical procedure in dentistry and you need not be worried about it.
Next Steps
I would suggest to go for a 3D xray ie CBCT which will show exact few mm approximation between tooth and nerve.
Based on the X-ray provided, the wisdom tooth appears to be impacted/malpositioned. In most cases, root canal treatment is not the preferred option for a wisdom tooth with deep decay and improper eruption, especially if the tooth is difficult to clean, has limited functional value, or is likely to cause recurrent problems.
However, a definitive treatment plan cannot be decided from this X-ray alone. A clinical examination, assessment of symptoms, extent of decay, and evaluation of the tooth’s restorability are required.
Next Steps
If the tooth is severely decayed and impacted, surgical extraction is generally the more predictable long-term treatment. Please consult an Oral and Maxillofacial Surgeon for a detailed evaluation and treatment planning.
