Breast cancer metastasis to bones, lungs, brain and liver. So before starting of treatment patient should undergo whole body pet CT scan and confirm the metastatic sites. If in any organ disease is present check for ER,PR and Her 2 status and if metastatic to knee only go for palliative radiotherapy to relieve pain and prevent fractures.

Please share the PET CT report. Old age is known for weak bones and fractures. Even inflammation or infection might show positive on the scan. If it multiple sites spread then she will require palliative treatment and supportive treatment. If it is single site then requires surgery.

Usually acute pain and swelling are features of inflammation. But if in pet scan ,SUV max is high and suggestive of metastatic disease ,then treatment should be started.
If there are no metastases anywhere in the body except for bone ,then Inj Zoledronic acid monthly or Denosumab monthly can be given ,provided the kidney function is normal.
