Mental wellness related problem!
Good evening psychiatrists or psychologists Doctors! I am very glad that after searching for a lot of online consultation apps, I have found a genuine one where the Doctors really give the answer to every question asked by the patient! I hope I will get my answer as everyone ASAP! Basically, I am Piyush Sharma and I am 18 years old and I have been suffering from a severe depression when I was 15 years old, and I have consulted with a lot of psychiatrists and they didn't find the solution to what I have been suffering with! So, When I was in 10th standard, I was constantly taking a lot of tension just because I hadn't been able to get the type of marks that I had been expecting... The main reason behind not getting the marks is due to the habit of procrastination... I have a big story to tell but due to the word limit, I just can't! After taking anti-depressent for 1.5 years, Now, I think Sometimes I feel a lot of happiness and sometimes have been in a very depressive state!

You're stuck in a loop most people never name out loud: the task feels huge, so you put it off, then the guilt kicks in, and that guilt makes the next thing feel even harder to start. Depression basically drains the fuel you'd need to break that cycle. So no, it's not a willpower issue. Honestly though, the loop itself is exactly what a psychologist can help you untangle. You've spent a while collecting opinions online might be time to actually book that session and talk it through with one person who gets the full picture, instead of piecing together answers from a feed.
Next Steps
Consult a Psychologist for proper diagnosis and treatment

Start very small, do your tasks for 5mins

Hi, I understand what you’re going through, Piyush. You’ve been dealing with depression since your school years, along with academic pressure, high expectations, and procrastination. Procrastination can sometimes create a cycle of delaying tasks, feeling guilty about it, and then becoming more stressed, so it’s important to approach it with patience rather than self-criticism. The changes you mentioned, sometimes feeling very happy and at other times becoming quite low, are also to be understood more closely. Notice when these changes happen, how long they last, and whether your sleep, energy, thoughts, or routine change during those periods. Try breaking your studies into very small tasks, starting with just 10–15 minutes, and keeping a simple mood and activity journal. Since you have previously taken antidepressants, don’t stop, restart, or change any medication on your own without medical guidance. Remember, Piyush, you don’t have to change everything at once. Small, consistent steps can gradually help you feel more in control and hopeful about your journey.
Health Tips
A few simple things you can try: • Do 20–30 minutes of exercise- physical activity each day like walking, cycling, stretching . • Practise deep breathing for 2–3 minutes when you feel overwhelmed. • When you notice yourself procrastinating, use the 5-minute rule and simply begin the task.

Absolutely the diagnosis in your case seems to be doubtful. As per your few details and symptoms..it seems to be performance anxiety which is leading to various psychological changes. It needs to be treated asap otherwise it may get complicated and can affect your personal, scholastic and social life. It can be well treated with counseling sessions and homeopathic medicine effectively and without any side effects. You need an expert psychologist who is a good homeopathic physician and counselor.
Next Steps
I have been working as a Homeopathic Psychiatrist and Counseling psychologist for the last 17 years of experience. You can contact me through an online appointment for further assistance.

Hi Piyush, thank you for sharing your concerns with the mental health professional forum. After reading your query, it seems like you've been dealing with mood related issues since your teenage years. There is also presence of high expectations of self and not able to achieve them along with procrastination. This feeling can often lead to emotional dysregulation and altered mood states. There is also mention of taking Psychiatric medications as well to help with mood. As a Clinical psychologist, I'd try to understand initial symptom development history, possible causes for mood fluctuation, and help you develop coping strategies for low mood and procrastination. Please make sure you're sleeping well to help with biological rhythms. You can book session with me or howsoever it suits. Please take timely help.

Take long walks: Nothing can be more therapeutic than going for long walks, especially in the morning. Avoid dark films and TV: This one can be hard because some of the best pop culture today is dark: Game of Thrones, Daredevil, The Man in High Castle. Movies like The Revenant, Split etc. Avoid dramas and crime movies no matter how critically acclaimed they are. What you should watch is comedies and coming of age movies and shows that elevate your mood and make you motivated. Watch Friends, This Is The End, It’s Kind of a Funny Story etc. Seek a therapist: Nothing will help you get you get of depression quicker than going to a therapist. Listen to meditation videos on YouTube: This is another great way to get more positive. Have a routine: Depression make you lazy. Having a routine will not let you be. Seek help from your loved ones and spend time with them: This is extremely important. Talk to your loved ones and tell them what you feel. Don’t keep anything inside. Avoid toxic, negative people and situations: If it’s your job, then you need to quit it. If it’s your relationship, then you need to separate.

Piyush, what you have been going through since your school years sounds quite distressing. When academic performance becomes strongly connected with our self-worth, pressure, guilt and constant overthinking can create a cycle where procrastination increases, expectations feel heavier, and eventually mood starts getting affected. So it would be important to understand the complete pattern rather than looking at procrastination alone. The periods of feeling very happy followed by feeling significantly low are also worth discussing in detail with your treating psychiatrist or psychologist. The duration, intensity, sleep, energy, thoughts and behaviour during these changes are important to understand before drawing any conclusion. Please don’t stop or change your antidepressant on your own. With proper assessment and therapy, the underlying patterns around perfectionism, self-criticism, academic pressure and procrastination can be worked on gradually. You don’t need to explain your entire story in one go. Start with what is troubling you the most right now, and we can understand it step by step. Piyush, this can become a cycle: high expectations → delaying the work → guilt → more stress → feeling low → delaying again. Therapy can help you break this cycle and learn practical ways to manage both procrastination and mood changes. Since you’ve been struggling with this for a few years, please consider taking therapy with me. We can work through it step by step.

Rather than assuming that everything is simply depression, I would first like to understand your experience in more detail. We would look at your mood, sleep, energy, concentration, interest in things you usually enjoy, thoughts about yourself, academic pressure, procrastination, and how all of this is affecting your day-to-day life. I would also like to understand what you mean when you say that sometimes you feel very happy and at other times very depressed. We would explore how long these changes last and whether there are any significant changes in your sleep, energy, thoughts or behaviour during those periods. This helps us understand the symptoms properly rather than jumping to a diagnosis. Since you’ve been taking an antidepressant for around 1½ years, I would also encourage you to have a review with your psychiatrist regarding how much it has helped, whether you are still experiencing significant symptoms, and whether any further assessment or treatment adjustment is needed. Please don’t stop or change the medication on your own. We can also work specifically on your procrastination. I don’t want you to simply think, “I’m lazy and that’s why I’m failing.” Often procrastination becomes a cycle of pressure → avoidance → temporary relief → guilt → more pressure. We can work together to understand what is happening for you and gradually break that cycle. -
Next Steps
Book appointment with me, I will want to ask more questions and discuss the next steps.

Your mood swings suggest you might need therapy (like CBT) added to your treatment plan. Please discuss with your psychiatrist. Also track your mood on a daily basis
