Low Haemoglobin and Rbc count
My father was suffering from blood in stool last week for 2-3 days and due to this his Haemoglobin level is reduced,  feelings extreme weakness, shortness of breath while doing day to day activities and his skin and nails look pale. After consulting dr he is taking iron and folic acid supplements along with his regular medication as he has stent in his heart.( Ecosprin is stopped by his dr for sometime).But even after dietary modification and oral supplements his symptoms are not improving.I am attaching his report. Please suggest what can be done as he is becoming very week.

It is only a CBC report. Please also get his Vit D3 and B12. Also his LFT, Sugar profile, Thyroid profile, Lipid profile. His kidney is working at a subnormal level.

If the blood shading with stool stop then weakness and fatigue will go within a month.. but be sure that the weakness is not because of any other physical issues like cardiac issues.

Identifying the exact cause of bleeding is important .. stopping ecosprin is not advisable unless the cause of anaemia is bleeding ulcer in the stomach

Ensure that the cause of blood loss is investigated properly and treated. Supplement with dietary fibre-chia /flax seeds. Consume iron and B12 rich foods-meat,green leafy vegetables etc. Medication- TAB. 2B12 Once a day TAB. Orofer XT Once a day If there is no improvement even after 3 to 4 weeks consider iv iron sucrose at hospital.

But we can try some iv injections if it's persistent anemia even after tablets and dietary modifications. But treating the cause of the anemia remains our mainstay treatment . Mention 1. the age of your father 2. whether this blood is bright red( fresh) or off red 3. Is defecation painful or not . 4. Add albendazole 400mg for 3 days , single dose at night. 5. Is there any history of hard stool or not

Dr sankar santhosh here , Firstly oral supplementation that the patient has been started on will take a couple of weeks for the patient to show improvements in symptoms , as hemoglobin tends to rise at a slow pace ….

Continue the oral medications for the time being , and withhold the ecospirin I would advise you to have an endoscopy and a colonoscopy to be done to rule out any gastrointestinal causes for the bleed .

Iron profile,  vitd3  advisable. Haem up cap one at night for 2 months after dinner. Stool color will change. Don't worry

You need to check the cause of blood in stools as it is possible that bleeding is still there but not visible to naked eye. It is usually not possible to have such a low Hb with small blood in stools for 2 to 3 days.

Patient can have 2 things, if the blood in the stool is red bright, this is hemorrhoids If the blood is dark/maroon, this can be due to ulcers in stomach or duodenum, for both of them, you urgently need to see a general surgeon atleast. Please go to a surgeon as early as you can. Get well soon.

Looking at his hemogram, I can see that he doesn’t have any iron deficiency or any blood related disorder which would lead to his reduced Hb. Which means the issue is related to the gut. And since he has a stent I don’t think stopping Ecosprin ( which is the blood thinner given so that blood does not clump in that stent) will not do any good to him. I would like you give me a little bit more history like if he vomited? If yes was there blood in it. Any stomach aches? If yes where? Also the quality ( how much blood, constipated or watery stool) of stool he passed. Has it happened before? Has he undergone any surgeries? Or is there any other medical condition associated? Also tell me what other medications is he on currently or previously.
Next Steps
To clearly know the cause, a regular stool examination will be required which includes stool OB(occult blood). Along with platelet analysis test that is PT-INR for that blood clumping part. Also he should undergo a colonoscopy if stool examination comes positive to confirm the actual cause

He might be needing a transfusion considering a quite low Hb. But for that a fresh CBC report would be required.
