It’s due to severe periodontitis condition which causes loss of tooth attachment fibers and leads tooth mobility.you need deep cleaning of teeth and gums followed by perio plastic surgery tissue and bone graft.
The reason of looseness of teeth which has plaque and receding gums can be because of periodontitis (infection in your gums and bone)
Next Steps
The first step is a complete and deep cleaning of teeth .
Health Tips
There are also other factors ( eg. occlusion, family history etc) responsible for loosening of teeth along with plaque . You need to consult a periodontist for complete evaluation.
