Your symptoms suggest likely acid reflux (GERD) or positional reflux. This happens due to acid movement and diaphragm sensitivity. Cardiac cause is less likely if no severe pain, sweating or radiation
Next Steps
-Pantoprazole 40 mg once daily before breakfast for 2 to 3 weeks
-Levosulpiride 25 mg once daily before dinner for 2 weeks
-If symptoms persist or worsen, ECG and chest evaluation can be considered
Health Tips
-Avoid lying down immediately after food (wait at least 2 to 3 hours)
-Sleep with head end elevated
-Avoid spicy, oily food and caffeine

There could be acid reflux or hiatus hernia. Please get endoscopy done to know the cause.
Take small frequent meals, avoid heavy meals, late evening meals, fatty, spicy meals, excess of tea, coffee.
Based on your history of acidity and palpitations, there are a few common reasons for this:
Positional Acidity: Even though you take medication, lying on your left side can sometimes cause stomach pressure that feels like nausea or 'vomit feel.' This pressure can also push against your diaphragm, making it feel harder to take a deep breath.
Heart Awareness: When you lie on your left side, your heart is physically closer to your ribs. It is very common to feel your heartbeat much more strongly in this position. For someone who already experiences palpitations and panic attacks, this sensation can feel like a 'breathing problem,' even if your lungs are functioning normally.
The Feedback Loop: The discomfort of the 'vomit feel' can trigger your body’s stress response, leading to a mild panic sensation that makes your breathing feel restricted.
Suggestions for Relief:
Try the Right Side: If the left side is uncomfortable, try sleeping on your right side or propped up slightly with 2–3 pillows (inclined position).
Meal Timing: Ensure your last meal is at least 2 hours before you lie down to reduce the 'vomit feel.'
Thyroid Check: Since you are on thyroid medication, it is worth checking your TSH levels. If your dose is slightly high, it can worsen palpitations and the 'on-edge' feeling at night.
Note: If you experience any sudden, crushing chest pain that spreads to your jaw or arm, or if the breathing difficulty does not go away when you sit up, please seek emergency care immediately.
Discomfort when sleeping on the left side can be caused by acid reflux or a hiatal hernia, which allows stomach acid into the esophagus. Other causes could be heart pressure, pericarditis, respiratory issues like asthma or a lung infection
Next Steps
needs further evaluation, try these two steps Sleep using a wedge pillow or extra pillows to keep your chest above your stomach. Switch to your right side to help reduce pressure on your heart and make breathing easier.
