When did you check your shoulder recently? If it was long back then you need consult a Physiotherapist or Orthopaedist for complete assessment and treatment.
Based on your injury the treatment protocol varies it may last from few days to months.
4 years is a very long time
Most probably the condition must have changed drastically over the period
