Knee pain since 10 years
I used to play volleyball/badminton since childhood. From last 10 years I have some instability in left knee and severe pain from last 2 years (specially in winters). Cracking sound always comes from knees, while walking around 7 crack sounds in 10 steps. While sitting, in around every 2 mins I feel knee joint is locked, then I have to straight the leg and a loud cracking sound comes. I had done MRI two times this year, which I am attaching. Please suggest some treatment advice.

The problem is due to quadriceps weakness and at the patellofemoral joint. You have patellofemoral malalignment causing instability and pain while doing provocative activities.

Dear friend, The problem as you stated is due to your quadriceps muscle weakness. It is important to understand that we are not treating your MRI but your symptoms. The symptoms due to quadriceps weakness are: 1. Cracking sounds from knee 2. Instability especially when you are walking downstairs 3. Pain while getting up from cross legged sitting or squatting or getting up after sitting for a long time. 4. Pain while climbing stairs. I think all this fits description of your problems. You definitely have some cartilage damage but that is a sequel of your muscle weakness which if ignored will cause more damage.
Next Steps
Treatment 1. Avoid activities which are aggravating the pain. 2. Strengthen your Quadriceps and increase your quadriceps muscle bulk. 3. No knee caps as it will weaken your muscle 4. Do Static Cycling which will help to tone up muscle. 5. Cool pack application after exercise Its is going to take 3 to 6 months for the pain to gradually reduce and gain strength to help you heal.

The problems which you are facing are mostly due to the meniscus injury and cartilage defect in patella with obstruction of movements at knee joint due to the same.
Next Steps
Usually conservative management is only required for healing of the defect using knee support, hot fomentation,  analgesic with calcium and vit d supplements. If there is no relief at all, toi may require Arthroscopy with joint debridement surgery.

Avoid heavy work,  avoid sitting down cross leg,  avoid Indian toilet..
