Intermittent tiredness/drowsiness
I am 42, male. For the past few months, I sometimes get sudden tiredness, drowsiness and a heavy feeling for a few hours. The strange thing is that I may be completely normal and active for several days, then suddenly it happens again. During the episode: BP is normal. Pulse is normal. Blood sugar is normal. I am not physically weak and can still walk/exercise normally. No unexplained weight loss. ECG, echo, TMT, brain scan and recent blood tests have been normal. Has anyone experienced this kind of intermittent tiredness/drowsiness? What was the cause in your case?

Intermittent episodes of tiredness and drowsiness can occur due to poor sleep, sleep apnea, thyroid dysfunction, anemia, medication effects, or other metabolic causes. Normal initial investigations do not rule out all possibilities.
Next Steps
Please consult  for detailed evaluation, including sleep history and review of previous reports. If episodes are associated with confusion, fainting, chest pain, or breathlessness, seek urgent medical care.

With your heart, brain and metabolic tests all coming back normal, episodes like yours often trace back to a sleep problem such as mild obstructive sleep apnea, even when there's no obvious snoring. A thyroid or vitamin deficiency can also cause this kind of on-and-off daytime drowsiness.
Next Steps
Ask your physician about checking your thyroid and vitamin B12 and D levels if these haven't been tested recently. A sleep study is also worth considering, since poor-quality or fragmented sleep often shows up as exactly this kind of drowsiness.
Health Tips
Please see your doctor sooner if these episodes get more frequent, last longer, or come with confusion or trouble waking up.
