Hello, I am Dr. Romain Rajan, General Physician (Ex Govt Medical Officer at RML Hospital, New Delhi), and your case has been allotted to me. A temperature of 103 to 104°F with headache, vertigo and vomiting should not be managed only with home remedies. Acidity alone would not usually explain such a high fever. She should be medically assessed, particularly if the fever remains this high or is associated with worsening headache. What you can do now • Keep her well hydrated. Give water or ORS in small, frequent sips, especially after vomiting. • Use light clothing and keep the room comfortably cool. Lukewarm sponging can be used for comfort. • Regarding the paracetamol 500 mg, because she vomited immediately afterwards, absorption is uncertain. Do not repeatedly redose without medical advice, as excessive paracetamol can be harmful. For most adults, doses should be separated by at least 4 hours and the maximum daily dose should not be exceeded. • Avoid self starting antibiotics, ibuprofen or multiple combination fever medicines until the cause is assessed. Please seek urgent medical care tonight if • Fever remains around 103 to 104°F despite measures • Severe or worsening headache • Persistent vomiting or inability to drink fluids • Increasing dizziness, confusion or excessive sleepiness • Neck stiffness, sensitivity to light or a new rash • Breathing difficulty, fainting or seizures High fever with significant headache and vomiting warrants proper examination rather than assuming acidity. Please share her age, current temperature, blood pressure, pulse, whether she has chills, abdominal pain, urinary symptoms, recent travel or mosquito exposure, and I can guide you further. For personalised one to one consultation, you can reach out to me directly on WhatsApp at 80762047 three eight. Wishing her a speedy recovery. Dr. Romain Rajan General Physician

103–104°F fever with vomiting in a 13-year-old warrants same-day medical evaluation. Give ORS/fluids and weight-based paracetamol; avoid repeat dosing immediately after vomiting. Seek urgent care if drowsy, confused, dehydrated, severe headache, neck stiffness, seizures, or breathing difficulty.

What's your child's weight. Use paracetamol at 15mg per kg weight dose.....give it every 6th hourly. You can also use ibugesic or mefenamic acid alternatively with paracetamol. Dose again depends upon weight Tepid sponging will help. Looks like viral fever. They start with high temperature and slowly come down.
Next Steps
feel free to consult me on practo for further evaluation and management

Give plenty of fluids/ORS in small frequent sips. Light clothing and lukewarm sponging may be done if required. Paracetamol can be continued according to weight-based dosing; do not repeat the dose immediately after vomiting. Avoid ibuprofen/aspirin for now. As fever is 103–104°F with headache and vomiting, please get her examined by a doctor today. If vomiting persists, she becomes unusually drowsy/confused, develops severe headache/neck stiffness, breathing difficulty, rash/bleeding, or is unable to take fluids, please visit the ER immediately.

The symptoms are highly suggestive of tropical fever which might not subside by taking just Paracetamol. It needs further evaluation like fever profile, ESR, CBC. So I would highly recommend you to kindly consult a doctor in person.

The symptoms point towards a probable tropical fever like syndrome. Could be Dengue most likely, Malaria and Leptospirosis. Do not wait at home, visit a MD Physician. Things can go bad rapidly. Till then give her atleast 4-5 litres of water per day and Give her Paracetamol every 6 hourly.

103-104 degree fever is high for a usual infection. Get a fever profile done- dengue malaria typhoid tests. Sponging will help. You can repeat paracetamol within 6 hours. But do not take more than 3 in one day. And keep a gap of minimum 6 hours. Ensure she has food in her stomach before paracetamol. Do not give empty stomach. Few biscuits are also fine.
Next Steps
Take ORS instead of regular water. Make sure it has label of WHO on it. Avoid sugary drinks. You can also take some coconut water. If vomiting doesnt stop please take tab ondansetron 4 mg once.
Health Tips
Contact doctor if symptoms worsen. Get a Fever profile done.

I’m concerned about 103–104°F fever with vomiting and dizziness in a 13-year-old; this needs prompt in-person assessment, not just a home remedy. Give small frequent sips of ORS/water and light clothing. Paracetamol should be weight-based (10–15 mg/kg per dose); dont repeat early or combine cold medicines.
Next Steps
Please seek urgent examination today, especially if vomiting continues. Go to emergency care now for repeated vomiting, drowsiness, breathing trouble, stiff neck, severe headache, rash or reduced urine. You may book a Practo consult or visit me at Synergy Multispeciality Hospital, Miraj.
