An ESR of 74 mm/hr combined with a hemoglobin of 11.3 g/dL â even without any obvious symptoms â is a significant finding that should not be ignored. ESR is a non-specific but important marker of inflammation or disease activity in the body. An elevated ESR with mild anemia together can point towards several conditions that need to be systematically ruled out: iron deficiency anemia (which can itself mildly raise ESR), autoimmune conditions like rheumatoid arthritis or lupus (which can be silent early on), chronic infections like tuberculosis (which can be truly asymptomatic in early stages and only show up on tests), inflammatory bowel disease, and â importantly â certain malignancies like lymphoma or multiple myeloma, where elevated ESR and anemia are classic early findings. The absence of symptoms is reassuring but does not rule these out.
Next Steps
Please get the following tests done: CRP (C-reactive protein), iron studies (serum ferritin + serum iron + TIBC), Vitamin B12, peripheral blood smear, kidney function tests, liver function tests, urine routine, chest X-ray (to rule out TB or any lung finding), RA factor and ANA (to screen for autoimmune disease). Based on age, also consider a serum protein electrophoresis (SPEP). All these results together will help narrow down the cause. See a physician in person with all the reports. If no other problem, as Haemoglobin improves esr will reduce. It is a non specific test.
High esr is due to infection or inflammation and it can be early marker of hypertension in future.
