Ciplar LA 40 is a long-acting propranolol - it is designed for once a day, so taking it twice is not the right way to chase a BP of 180/110. Yes, it can pull the pulse down too much, and with CTD-T-AM (telmisartan + amlodipine + chlorthalidone) already on board, propranolol is a poor add-on for pressure control anyway; it is mainly for tremor, migraine, palpitations or after a heart attack. A BP of 180/110 at 59 means the current combination needs re-titration by a physician, not a self-doubled beta blocker. Do not stop propranolol abruptly either - it must be tapered.
Next Steps
Keep Ciplar LA 40 once daily, check pulse at home (if under 55/min or you feel giddy, hold it and inform your doctor), and note BP twice daily for a week along with a kidney function, electrolytes, ECG and urine test. Bring that record to a physician consult - on Practo or my OPD at Synergy Multispeciality Hospital, Miraj - and we will fix the combination properly. Chest pain, breathlessness, severe headache or slurred speech at these readings means go to emergency now.
Taking ciplar 40mg twice can lower pulse to dangerous levels. In place of ctd am( containing telmisartan, amlodipine, chlorthalidone, I.e., 3 different molecules in one pill), take molecules separately so that titrating dose, as per b.p. levels is more convenient.
Please share details of your b.p. problem, then I can help you further.
Requires a proper assessment to understand the underlying cause and guide you.
Please book a consultation for a detailed evaluation, personalized treatment plan and further advice.
Some times can be happen, but Ciplar LA 40 is not cardio selective , any other side effects it can cause , better your drugs has to revise again, kindly take consultation
