First- how did you check the BP. (Manualy or automatic one).Because sometimes your automatic one will.pick your heart rate and shows the according to that.
Second thing is that, before checking BP he should be at rest for at least 10-15 mins and relaxed, avoid taking coffee 30mins before checking, and at first if you find the BP high don't panic, tell him to relax and recheck after 15 mins.
Third point is that, in elderly who are on antihypertensive BP of upto 150/90 is acceptable
Fourth thing is life style modification
After all this still the BP is high, then you should consult doctor for changing medicine
